Just you, no guilt about DH, the DCs, your Mum.
Can you write it down, get things out of your head a bit?
Also, maybe think in chapters. Next 6 months, next year, 3 years, 5 years, 10 years. Things can change. What worked well enough in the past is evidently not working today. The solution you find for the next five years may no longer work in ten.
Then look at what's non-negotiable, important, nice to have, wishful thinking.
For example, your health and well-being and that of your family are the absolute priority and non-negotiable. This includes a healthy, loving relationship with yourself (less of this guilt), your dh (true respect and equality) and your dc (the only thing they need really) .
A safe place to live, with good enough schools for your children, a place where enough of everyone's needs are met.
Jobs (for you and dh) that pay the bills and allow you to enjoy life but don't lead to burnout. You not having to bear the financial burden in such an unbalanced and harmful way. Both of you accepting the implications of this and working together to find solutions that protect the non-negotiables.
Nice to have but not essential would be the particular type of house and neighbourhood. Your children staying close to their childhood friends. High-paying jobs. Top schools in pricey catchment neighbourhoods. Foreign holidays. Recent cars.
I can almost hear you saying "if only he earned more...", or "if only we lived in my hometown....it would all be OK". Wishful thinking would be wanting your DH to earn far more doing his creative job, or deciding to become an investment banker and taking on the role of main provider. Not going to happen.
Moving to your hometown, though, could be a valid solution. But it may not be the only one.
Again, I think you might benefit from some sessions with a therapist to talk things through and make these choices from a position of strength.
