Only way I can lose on Mounjaro is to eat no more than 500 cal and exercise

Hello everyone
I have been on MJ for 2 1/2 months. I have very good loss in the first five weeks I lost about 1 stone. I carried on the second month and lost 4kg. Then everything came to a halt. 4 weeks of losing nothing.
I upped my dose to 3 mg and still had great appetite suppression. I have always eaten about 900 cal a day forcing myself. I dropped my calories to 800 and then 700 still lost nothing. The only way the scale started to move a little bit is by dropping my calories to 500-600. I am still doing 10,000 steps a day.
I still have about 3 stones to lose to get to healthy weight and I can’t carry on eating so little. Even though I am not hungry, It is not healthy.
I take v low thyroid meds dose but my TSH is fine now and T4 which is low is within rage so no way the doctor will up my dose. I don’t think I have a whoosh effect coming either. I also took 2 days re feed and ate at 1100-1200 cal a day and that did nothing.
Everyone here says to up the dose when the appetite suppression goes but I have good appetite suppression. No sure what to do.
Grateful to anyone for suggestions that works or anyone with similar experience and what they did to get losing again.

The medication doesn't work solely by reducing appetite, so by not increasing the dose when it stopped being effective, you have likely hindered your progress.

I moved up in very small increments myself, but I could do this as I made tiny increases that were enough to allow the medication to continue being effective if progress started to stall.

Your weight may be increasing a little for other reasons such as menstrual cycle, hot weather water retention, muscle gain, etc.

It's not unexpected that you aren't losing weight while continuing to take a partial dose.

Thank you so so so so much everyone.
To clarify: I eat 80 grams of lean protein a day sometimes 100 grams and drink 3 L of water and stay low carb. No alcohol at all. I count my calories meticulously so I am sure about the calories intake. Yes, the walk burns about 300 cal. I am 3 month 1.5 wk in so have been trying different strategies the last 4-5 weeks. Nothing worked until I dropped my calorie intake to 500-600 cal as an experiment. It is too unhealthy and I am sooo tired even though I am not hungry.
I had yo-yo dieted over the years and that certainly messed up my metabolism but had hoped the MJ will help with this.

I stayed on 3 mg as I have v good appetite suppression and everyone’s advice on MN was to go up the doses slowly and only if the current dose doesn’t work. I also read here on MN that people who went on higher doses didn’t necessarily lose more/faster if they had good appetite suppression on the previous doses.
I am 13st 5 lb and 5 6 tall.

I have NO intention on staying at such low calorie intake. It is too unhealthy and that why I am asking for help.

How are you eating that much protein for that few calories please? I’m terrible at getting my protein up even though it feels like that’s all I’m eating!

If you’re lifting weights 3 times a week, you could be gaining muscle and losing fat. So the scales stay the same, however you are actually achieving your goal.

OP is eating whey protein powder, prawns and skyr. The powder gives you about 20g and you can shake it with water. It doesn’t seem a great way to go about meals, though.

She's not gaining muscle in such an extreme calorie deficit. It's impossible. Even with plenty of calories or excess body fat to fuel muscle growth for a woman it's only ever a very small amount, nowhere near equivalent to the amount of fat lost.

Prior to the extreme calorie cutting, she felt she was plateauing so cut calories. Sadly she’s likely lost any muscle gained now.

Junglekat · 10/07/2026 18:21

Maybe it’s because you’re taking the medication not how it’s been prescribed. Go back to the prescriber for support?
ideally stop the medication - because with such a calorie deficit you’d lose weight anyway.

If she’s only eating 750 cals then the dose is irrelevant.

@PositiveEnergyyy if your TSH is normal then that’s the end of the pathway with your GP. She’s correct.

Not really as gaining 1kg of muscle is a massive endeavour. It’s impossible (unless on steroids) to be gaining muscle at the same rate as loosing fat. However, for 2-3 weeks it could be water retention.

What was it. 4 is normal but some have symptoms. Optimal is between 0.5 and 2.

MxCactus · 11/07/2026 08:51

How long have you been not losing OP? Because of my menstrual cycle I have periods where I diet but still gain on the scales. I also followed a fitness influencer who was very strict on her diet and she gained weight for a month with a high deficit! Before suddenly dropping a load of weight.

She explained that even on a diet the loss won't be linear, but you need to just be consistent and keep going and over months the weight will steadily decline, despite short term gains when you're dieting.
