3.75mg - is anyone doing this?

Very reluctant to go straight from 2.5 to 5 as I have a few things on during this time and don't want to be ill.

Is anyone doing the twists for 3.75, and if so, can you explain how to do this?

Me 🙋 you turn the pen 45 clicks rather than the full 60 clicks that is a full dose.

I took the bonus dose from my first pen which worked out as 3mg and I had side effects so was too afraid to increase to 5mg.

So instead, I did 3.75mg - have taken my fifth dose from that pen last week. Was extremely effective for the full seven days until this 5th dose - definitely hasn't been just as good this week.

I plan on taking 3.75mg of my new pen this week, if suppression still isn't great then I will increase to 5mg the week after. I want to give it one more week in case this pen had lost effectiveness due to me using it for a 5th week of 3.75mg (perhaps the pen was starting to go 'off'? Who knows). I didn't keep it in the fridge this time after the first dose as I went on holiday.

I had diarrhea 48 hours after my injection on weeks 1, 2, 3 and 5 of this pen! Albeit from week two it was one bout and no tummy pain. So I'm not dying about increasing my dose.

I did that for a few weeks too actually. You simply count the clicks - as the poster above mentioned. I didn’t have any side effects working my way up to 5 mg this way.

Me - went up to 3.75 on Saturday after 2.5 for 4 weeks. No regrets - the side effects the day after were still pretty powerful (but then subsided), and I have good suppression so far. Will see how I feel at the end of the week, if I want to stay on this or go up to 5.

I've had 6 weeks of 3.75mg now and it is working well for me.

I inject on a Saturday afternoon, feel a bit tired Sunday evening and very low appetite Monday-Wednesday-ish. Thursday/Friday ok suppression and I can eat reduced portions but nearly normally Saturday/Sunday - but over the week I keep within my weekly calorie deficit.

The 45 clicks also mean you don't need to hard twist for the 5th dose, but it is very tempting the 6th one that's left (but it is beyond 30 days).

3.75mg is more than the 2.5mg dose on a 2.5mg pen, though. So you'd need to do two separate injections - one at the full dose of 2.5mg and 60 clicks then another injection (with a new needle) of 1.25mg so 30 clicks.

If you have a 5mg pen then for 3.75mg it would be just one injection, with 45 clicks.

I did this on what should have been my 4th 2.5mg dose - wasn’t getting bad effects and barely any supression on 2.5mg and was concerned about the jump, so I took my 2.5mg 4th dose and then the extra 1.25mg with my ‘bonus’ dose - 30 clicks.

i had a touch more side effects - nothing major just a little more nausea. And then found I had some proper supression for days 4 and 5. On 2.5mg I found days 6 and 7 really hard - but this week its been a lot easier, although I still have hunger cues (which I want - don’t want total supression all week).

i start 5mg tonight and i feel positive that i can manage that jump this week.

If you want to get 3.75mg out of a 2.5mg pen you will need to do 90 clicks (two injections, two fresh needles) or extract 0.9ml using a 1ml syringe to get 3.75mg (full 2.5mg dose is 0.6ml).

You will probably only get 3 weeks out of the pen and if you use clicks will probably need to break the pen locking mechanism or use a syringe to get the full 3rd dose out (full pen contains 3ml so 3 x 0.9ml = 2.7ml)

It would be unwise to do this until you have completed 4 full weeks of 2.5mg as it is still building up in your system until then, but sounds like you have and are being sensible.

I have done 5 weeks of 2.5mg and 8 weeks of 3.75mg now and it is still giving me the level of suppression and steady losses (avg 2.3lb a week) I am happy with.

just ordered my 5mg pen and i’m going to do the 3.75mg dose as well. Been on 2.5 for the last 4 weeks and going to try get the 5th dose out the pen on Saturday then move upto 3.75.

if you do 3.75, how many doses do you get out the pen? I know it has to be used within 30 days of opening so can you just do 5 doses and you end up wasting a dose because the 30 days runs out? Thank you

I'm doing 2.5mg twice a week rather than 5mg weekly

3.75mg is 45 clicks in a 5mg pen. You will get 5 doses, always do each with a new pen needle, without having to use a syringe or hard twist.

There would, in theory, be enough for a 6th dose, you would get around 23 clicks without breaking the pen, then you would need to hard twist to get to 45 again (or use a syringe). Doses would be on days 0,7,14,21,28,35. Meaning 6th dose would be 5 days over the discard after 30 days instructions. Eli Lilly have remained consistent saying pen should be discarded after 30 days. It is up to you and your attitude to risk if you wish to use medication you inject beyond it's discard after date not knowing why it should be discarded - it may be ok, or it might be unstable/contaminated or just simply not as effective. Cloudy indicates a problem with the medication, clear does not mean it is safe.

If you are using 45 clicks it helps to mark the 45 click point in the window with a ball point pen the first time you do it to know where you are going to each time if you lose count!

I have read some providers, not all, challenge patients who are regularly not ordering pens on a 4 week schedule as adjusting doses is off license to what they should prescribe to.

Discarded medication is not "wasted" if you get 5 doses (already 1 more than you should have) and have the effect you wanted for those 5 weeks.

I don’t want to risk using it over 30 days so will defo discard at that point, i just wondered how much would be left over and end up being discarded with it not being cheap but i think its worth it as i don’t want to risk side effects and would rather go slow and steady!

I just did one week of 3.75 as a step up to 5mg.
It was fine with no side effects so I decided to do the full 5mg the next week and that was fine too for me.

So I ended up doing this and will take my last 3.75mg from my 5mg pen next week. There is alot leftover but the 5mg pen didn't cost me more than 2.5mg so not too bothered.

I will do the same with the next pen. The 3.75mg is giving me alot of suppression. Especially on days 3 and 4, so I don't think I should move up a dose quite yet.
