Hi 👋

A thread for anyone who wants to discuss their Create Fertility experiences.

I'm 38, currently in a 3 cycle Natural Modfied ICSI package and nearing transfer stage.

Update from me...looking like possible egg collection on Wed's, will know more Monday.

No transfer this time though, will have to be a FET as my lining may have a polyp on it and therefore a Hysteroscopy has been advised to investigate and if it is a polyp, remove it. A FET can then take place the cycle after, so possibly a FET may start end of Jan if I can get the Hysteroscopy done in time. I've done some reading and I understand if it is polyps then its usually that estrogen that triggers it...would make sense seen as I've had almost back to back cycles. Just glad it got picked up as polyps can cause implantation and miscarriages-just something else to rule out before transfer.

Has anyone had a Hysteroscopy?

@Sunflower360 i have a scan Monday and I suspect collection will be Wednesday. My estrogen levels are low and I need to up my dose again to 300 but I don’t have enough meds so he said 275 should be fine 🤞🏽 what does a hysteroscopy involve?

@Doxy21 ah, ok, so we may have collection on the same day! I'm still on 150 at the moment. I'm glad they did as my lead follicle goes crazy on highet doses.

Hysteroscopy is where they check the lining of your womb. They use like a small telescope thing with a light and camera and from there decide if anything needs removing like polyps. To be honest, if anything, it's another thing to rule out...albeit a costly one, but I would rather do it and give the transfers the best shot possible.

Could you not buy more stims, or was that a choice?

@Sunflower360 did they advise where to go for the hysteroscopy? I haven't done it before and was only told of it in our last consultation about the failed transfer. The doctor said the clinic doesnt do it and gave us an email address (NHS private) to reach out to. I'm just curious if the other clinics have the same approach?

Hi everyone. I met my original doctor today and glad I've changed back, feeling better.
Interestingly, he said its non-negotiable that I have to do every other cycle... I'm sorry you may have a polyp @Sunflower360 and fresh transfer is off. Are you doing the hysteroscopy at your clinic? Birmingham don't do them so I went with my miscarriage doctor under sedation. Will you grt it done before Christmas?

@VSRCM I went private for mine but am under a Reproductive Immunologist and his clinic do them.

I'll chase next week for my nurse consult to get my meds ordered but am on track for my end of December cycle. X

@Doxy21 50 off is not too much...seems a shame having to buy a pen for a small dose

@VSRCM I understand it is somewhere else at Create don't do it, pending details at the moment. Are you considering going elsewhere from the referral? How much was the one you have been signposted to and what are the wait times?

@VenusStarr did you have a Hysteroscopy? How did you find it? Very unlikely to be able to get it done before Christmas as I'll need to wait for my withdrawal bleed...I was hoping for the week after that if possible, so I'm keen to start looking into it and get it booked in if possible. Good to hear you have reassigned your previous Dr, so will you be having a break between cycles then?

@Sunflower360 I won’t be doing it in the U.K. We have enquired with the place Create recommended and the cost is £2,800.

The Prague clinic I’m enquiring with recommended something similar prior to the FET; so I’ll do it there. They also advised me to start taking oral probiotics so maybe consider that.

@Sunflower360 So NHS private quoted £2,225,
Spire hospital was £2,620 and Nuffield £3,230. Amazing (Frustrating) how the prices differ so much in the same area.

Yes, I've had 2 hysteroscopies @Sunflower360 one was with the nhs as part of my infertility investigations and they removed a polyp. The second was private because the ivf clinic thought I had scarring after my MMC surgery.

Yeh, I'll start end of December, then have to have a cycle off, then stim and collection, then a cycle off. This doctor said he wants to check my lining so might recommend freeze all for my 3rd cycle too if my lining is no good, so that's reassured me. He also said he supports me having lubion and cyclogest for transfer.

@VSRCM sorry was the private one via Create? £2225 was more what I thought it would cost, what was the turnaround with that?

@VenusStarr have you found the Hysteroscopies useful? Also what was the procedure like? Sounds like you have a good plan with your package, how did the lubion come about, was your Progesterone tested?

I had it under general anesthetic with the nhs and the polyp removal, the sedation with the possible scarring one. That showed nothing sinister, but I got them to check for endometritis while they were in there which was also clear. I don't think I'd want one not under sedation though.

I bled heavily in my fresh transfer then implanted late and my progesterone was really low even though I was on 600mg of progesterone and had 7 eggs collected so in my fet they put me on lubion and the levels were OK. He said it won't harm, so I'd rather try x

Where was the £2225 one? @VenusStarr thanks just thinking if I would need to consider lubion for a FET, suppose I would have Progesterone levels tested in advance... not had those done bar full usual day 21 tests when I had my NHS cycle which was around 30 I think... not sure if that is

@Sunflower360 The clinic are saying there's no optimum level of progesterone in a fresh cycle, especially as you should have the corpus luteum. But in a FET, definitely get your levels tested on transfer day, you won't be producing it if you do a medicated cycle. I had no bleeding in my last pregnancy and my levels were good. I'm convinced it helped implantation for me. The doctors main concern was the price but he said it won't harm so if it reassure me he's happy for me to do it.

In a natural cycle 30+ confirms ovulation. If I remember correctly you have had losses haven't you and are around 38? I think Tommy's guidance is to use progesterone supplementation in pregnancies regardless of natural or ivf.

@VenusStarr thanks. Yeah I had a chemical in the summer, so I'll be sure to push for a test then. I'm due to be having Progesterone pessaries Utrogestan (got that already), think twice a day, but I would imagine the FET plan will be set out once I get this hysteroscopy sorted.

Does anyone know, if you have a FET instead of a fresh transfer with Create within the 3 cycles what does that include? I've never had a FET so not sure what would need to be included but I would like to think it would include meds seen as it would not have been required in a fresh transfer due to the stims...
