What is a pelvic prolapse?

As the muscles, ligaments and supporting tissues in the pelvis become weaker, they are less able to hold in the organs of the pelvis such as the womb (uterus) or bladder.

Gravity pulls these organs down and, in the more severe cases, may appear through the entrance to the vagina.

A variety of problems can occur, depending on where the weakness lies and which organs are able to descend, but in every case there is some degree of prolapse of the vaginal wall, which begins to invert (rather like a sock turning inside out).
Prolapse of the womb or uterus is the most common prolapse, affecting as many as one in eight older women to some degree
Prolapse of the bladder, known as a cystocele, is less common.
Prolapse of the urethra (the tube that carries urine out of the bladder) is known as a urethrocele.
Prolapse of the intestines is quite rare, and known as an enterocele or rectocele.

Symptoms

Symptoms depend on which tissues descend, and how severe the prolapse is.

They may include:
A sense of heaviness or pressure in the pelvis.
The appearance of a bulge of tissue in the genital area, which can be quite alarming, and is often red and sore.
Urinary problems, such as having to urinate more frequently, feeling the need urgently, being incontinent (losing control of the bladder) or, conversely, being unable to pass urine when you need to.
Pain in the pelvis or lower back.
Sexual problems, including pain and decreased libido.
Constipation.
Vaginal discharge or bleeding.

Treatment and recovery

Once a prolapse has developed, surgery to fix the affected organs is usually the only way to cure it effectively.

However, another option is to use a device known as a vaginal ring pessary. This is rather like a contraceptive diaphragm or cervical cap. It's made of silicone or latex, and placed in the vagina to push back the prolapsed organs and hold them in place. Many women happily manage their prolapse this way.

Still pretty sore today. Trying to figure out whether I have a fecal impaction (requiring another trip to a&e Sad ) or "just" swelling.

Ed have you been able to rest a bit more or are you still training for the next Olympics? Wink

Hi just wanted to thank posters old & new for the wealth of useful information which I have found really helpful.
Just over the shock of returning from my gp referral hospital gynae appointment. Just interested to find out if everyone got their surgery appointment date & had to sign all paperwork agreeing to their surgery at this first appointment.
I was quite shocked, especially as they were keen to operate in July. (I've pushed for September as I can enjoy the summer holidays & get my DS & DD back to school first.)
I was told I had a large Rectocele (which I had worked out for myself) a smaller Cystocele & that my cervix has dropped. This may result in the need for a hysterectomy, which would be carried out the same time as the posterior repair. Apparantly once under general anasthetic everything relaxes & drops further! Has any one else been told this? I'd be interested to hear.

Hi, I am feeling exhausted! I have got such a busy week this week, got to take DS2 to the dentists after school today, then tomorrow he has got a kidney scan, luckily my DH is taking him to that, on thurs its his sports day (which I have told him I won't be able to go to) but we are going round to a friend's after school. Then on friday it's his 5th birthday and on sunday we are having a whole class party at a soft play centre. So I think I was just feeling a bit overwhelmed yesterday. I haven't got any help now, my mum lives 200 miles away and helped for 2 weeks but has now gone home, the in laws are in Australia for 2 months and our next door neighbours are on holiday. I have got one friend who is helping with dropping off and picking up DS2 from school but that's it!

Hopefully once I get this week out of the way it'll be a bit easier again.

missdiscombobulated - sorry I can't help with the tvt as I had a prolapse repair but I can tell you the recovery with young children is hard! I naively thought I'm only 31 I''m sure I will recover quickly and the consultant said the recovery time is 3-4 weeks but in the leaflet I was given it said 'after 3 months you should feel more or less back to normal', I think that is far more realistic!!

My DS1 has just fallen asleep 10 minutes before I have to go and get his brother from school, typical! So I have got a minute to spare.

dizzylicious I didn't have to sign anything at my 1st appointment. I actually went to see a consultant twice and the 1st time I went she did actually give me a date, it was last year and my DH who is a policeman was having to work the olympics and so I said I needed to know when it would be as I wasn't sure if he could get time off. In the end I cancelled the operation because he wasn't confident he could get time off and the 2nd time I saw a different consultant and he didn't give me a date, I only got a week's notice!

He didn't say anything about everything falling further under general anaesthetic to me but it sounds like quite a lot of work to have done. (I had an anterior and posterior repair).

neoprene - hope it's not a faecal impaction, that doesn't sound pleasant! hugs xxx

Dizzy - I had my first appt with gynae this time last year and was put on the waiting list for surgery then, warned it would be at least 12 weeks. I had the op on 27th Sept, think I had two or three weeks notice, I have to say it was perfect timing, my symptoms abated quite a bit and I was able to have a normal summer holidays with the DCs, get them settled into school and was back in action after half term, fully fit by Christmas.

MissDis - my friend had TVT two years ago, but the surgeon only agreed when she totally convinced him she won't be having any more children (she meant it).

Kotinka - the physio basically said I need to do the set PFEs which are usually recommended, ie 10x 10s squeeze 3 x a day (building up to the 10s), plus 10x fast squeeze 3 x a day, plus rapid squeezes at random as you would if you felt a sneeze coming on. She measured my strength and said I was only able to hold for 7s at the first appt, so I should start with that and build up to 10s. Also that I was stronger on one side than the other and had to try and squeeze that side more. Apart from that it was general lifestyle and exercise advice. I use an Iphone app to remind me to do the PFEs, it is called Kegel trainer, you set it at fixed times and it alerts you, then you programme in your squeeze/relax intervals and the display counts down as you do them.

Well,I didn't manage three times a day a lot of the time, but I really did try to keep to two lots. I have slacked off a bit again now, but I do lots of random squeezes both short and long and try to always engage the PF fully before lifting, pushing, pulling or otherwise exerting it. I really must get back to sticking to the regime though.

Ugh. Infection and lost stitch confirmed so wound is coming apart but they can't fix it until healed as the "tissue is now such poor quality". Sad

Getting antibiotics and Canesten.

Oh neoprene... What else can go wrong? You have had so many issues relating to your op, bet you are getting fed up with it all, and all you want is that it would get all sorted out, sooner rather than later. Be strong my dear

They kept me in hospital last night so I can see my surgeon this morning. This was probably a blessing as DD (3) has chickenpox so I probably wouldn't have got any more sleep at home. What else can happen?

oh neoprene at least you are keeping your sense of humour! I hope your surgeon can sort you out and am glad you got some rest in hospital. Let us know how you get on.

I am 5 weeks post op today and still haven't heard anything about a follow up appointment but the hospital I had it done at does seem to be very last minute with everything. I have still got the urgent bowel thing which is worrying me a bit as I don't think my friend is going to do the school run for me next week. If I told her she probably would but I'm a bit too embarassed. Oh well, I've only got 3 weeks and 2 days of school runs to do then it's the summer holiday!

Oh Neoprene what an awful time you are having, hope that your surgeon can help today.

MrsC 5 weeks post op and still having urgency I would give your consultants secretary a call and request a follow up appointment to discuss. You should not have to try and cope with that.

Mrsclairet I phoned harley medical last Friday to ask the surgeon about the urgency thing... Since last Thursday I'm having excruciating spasms in that area, feels like I have to empty my bowels all the time... Can't bend , sit, stand without it taking over my every thought m turning into a grumpy so-n-so :( anyway, surgeon said to stay on top of painkillers (paracetamol) as I've not been taking any since end if first week, and that spasms etc are normal for up to 8 weeks... Also to ease off the lactulose... But I feel so full down there it's almost as if I'm not emptying properly....

Neoprene I have a stitch hanging out its soooo painful when it touches the panty liners I'm still wearing (think I'm coming up to 5 weeks next Monday)... I thought my wound had come apart too as I'm still pushing to poop, don't know how else you'd go without actively pushing it out...

I can honestly say I was naive in that I didn't realise how long it would take to 'get better'... I'm emailing husband tonight the info leaflet I recently shared on here as I'm sure he thinks I'm making it up! (He was having a rant yesterday about the state of the house, I just had to lie on the bed and be out of the way.... He had one, yes one, day off when I came out of hospital.....)

Thanks for the replies & hope everyone is recovering today. Sounds like some of you have had a pretty rough time of it. Here's to the day when life is normal again, mind you once you've got children life is never normal.

claresavonrecruitment I can't believe your husband only had 1 day off!! My DH took 2 and a half weeks and literally completely looked after the kids during that time, he didn't do that much housework and I had to nag him to put clothes on to wash but apart from that was pretty good. As soon as he went back to work he seemed to start expecting me to do more but he hasn't moaned about the house yet. I think I was a bit naive too to be honest.

The urgency thing isn't too bad because it only happens once in the morning but I am not taking any laxatives and it feels a bit weird....it almost feels like I can feel things moving through more than I did before (sorry TMI) and sometimes it doesn't feel like I have finished. I think I will call the hospital and see if I can get a follow up appointment.

Sorry to hear you are having a bad time Neoprene - the stitch thing is what we are most frightened of happening. I hope it is nowhere near as bad as you think.

I have found the link for the pelvic floor website that my physio gave me. It is all about Pelvic Dysfunction which seems to be the in thing at the moment for explaining pooing problems, not sorted with the rectocele repair.

I have found putting my feet on boxes helps. The Americans have a special device for it : www.squattypotty.com/?Click=13560 but I find my two plastic boxes (one for each foot) fine. I wasn't impressed by the suggestion at first and immediately post op it was not comfortable. However, as the weeks/months have passed, it does help and it is easier to empty your bowels more fully, at least some of the time (I find a lot of things have helped, but nothing actually completely solves it all )

Also, the michelle kenaway site is useful and more straight forward.

I'm home! Finally got to see the surgeon and although a stitch has come out, she reckons it will heal ok and has packed me off with antibiotics. Pleeease let the worst be over now.
