I think that was what they did with small pox, wasn't it? Any outbreaks and they stepped up the vaccination in that area, so it worked for that one.

Only thing is that I wonder with 2-3 weeks before the vaccination properly takes whether it would be better to go for the surrounded areas to stop it spreading outwards.

Firefliess · 09/05/2021 12:47

It's also of limited use vaccinating around an outbreak if it's a school-based outbreak and you don't do under 18s

Had not looked at the rates in Scotland for a very long time. Not far off 100 per 100,000 in Moray, but as pointed out above the next is high 30s.

@Firefliess a lot of the cases I've heard about have been in the biggest high school and a few nurseries. So you are right, it might not make much difference. I just thought it was an interesting move!

I'm not so sure, to me that makes it more likely to be useful, since there will be more transmissions before more vulnerable get it, such that it's therefore more useful to vaccinate the more vulnerable as they'll be more likely to have had time enough to gain some protection from the vaccine. Whereas if it's already a community outbreak, more likely to be fewer chains to the vulnerable.

I'm not sure of the value anyway, encourage vulnerable hesitant people fore sure, but non-vulnerable, as you say it may already be too late.

Firefliess · 09/05/2021 14:28

Yes that's a good point *sirfred that picking on a local area and promoting vaccines in response to an outbreak might work for some people who are hesitant, as it gives a new and specific reason to reconsider a decision not to have it.

I see what you're saying about the vulnerable usually being a link or two down the chain from a school based outbreak. But by the time we've clocked that there's a big school outbreak, the school is probably going to be closed anyway, so it's already into community transmission. We can't easily get it under control in order to reopen the school safely if the kids aren't getting vaccinated. Getting people to come into vaccine centres in the midst of a big outbreak might not be such a great idea either - though I guess that's what we were doing everywhere in January.

Frazzled2207 · 09/05/2021 17:44

ok so the outbreak in Bolton appears to be gathering steam. However a lot of surge testing has been going on there since Friday. Would that account for the increase in positive tests do you know or will any positive surge testing be on top of the usual pcr results?

wintertravel1980 · 09/05/2021 17:57

Yes, Bolton is definitely a problem. The reported prevalence rate is currently 100/100,000 but it is likely to double based on the latest specimen date numbers. I assume results from the surge testing will be reported within the usual PCR numbers. It will be interesting to see the updated positivity rates. The latest number is 4.5%.

Zoe rate for the whole of UK also jumped again today - up by more than 10% in a single day.

sirfredfredgeorge · 09/05/2021 18:17

frazzled the surge testing should appear in the normal results, it always has before. However 4.5% positivity does not suggest that much surge testing yet if wintertravel's number covers the same time period.

Frazzled2207 · 09/05/2021 19:11

Thanks for that. There is definitely a lot of surge testing happening according to local media but it is very localised. Hopefully in the next few days things will level off.
Also Erewash looks worrying but I have to admit I just had to google where it is. (The bit between Derby and Nottingham).

25 Council areas with the highest rates in England.

Rates per 100,000 for each 7 day period from w/e 1st April through to the most recent largely complete period to 6th May. The final column shows the rate to May 8th (eg with incomplete cases), but only for those authorities where that is already higher than the rate to 6th May.

Erewash now close to 200 cases per 100,000. Largely driven by a school outbreak as I understand.

Bolton next at close to 140 cases per 100,000

Quite a large further gap to Blackburn at 80 per 100,000

Selby and Bedford then make up to top 5 in the 60s per 100,000. Clear downward trajectory in Selby, whilst although heading up Bedford's trajectory, at the moment at least, not at Erewash, Bolton, or previously Hyndurn levels.

Hyndburn appears in the next 4 and has shown a rapid decline from a peak of close to 150. Potentially positive for Erewash and Bolton if they follow the same track.

As the rest show rates generally have fallen significantly; the final three all have rates below 40.

Middle class Harborough, and South Northants interesting inclusions. Although Harborough had 27 cases in two days but none (yet) in the last 2.

Conspicuous by their absence, and again a positive, the likes of Leicester and Luton.
