What do you mean the front and back doesn't join up. Do you mean the bias binding or the fact that the front neckline is lower than the back (which is normal - check your store bought tops).
You don't need a yoke. You usually find them on shirts but they also add a decorative element to the garment. Also, just googled it and apparently they provide support for looser parts of the garment, which makes sense, I guess. Also, don't do two front pieces! The necklines, the armholes and sometimes the general shape will be different.
Instead, attach the yoke and the back pattern piece together - trace them onto another sheet of paper, remove the seam allowance between them, and join them together. Or actually, have a go at sewing the yoke, it's really easy. And you can use another fabric for the yoke to add some contrast.
Also the baggy t-shirt, when stitching in at the back, was there already a seam or did you just create one? Better to stitch in from the sides. Before sewing, you can double check your measurements. Measure across your bust and waist and see if the bust and waist measurement across the t-shirt is similar. The t-shirts measurements should be slightly larger as there'll be added ease, which is needed so that the t-shirt isn't skin tight.
