If we have more than two good quality embryos we will go for a double transfer. If we have two we will transfer just the one. we think we may have things to learn from the transfers so don't want to squirt all our eggs into one basket the first time over. If that makes sense.

I read that only about 40% survive to day 5 so we should have four I guess. Praying for more though obviously.

It can vary Boris 100% of mine have made it before and I've also had 50%. Yours are at a really good stage for day 3 so unless anything drastically goes wrong I reckon you are going to have a minimum or 4 or 5 nice blastos Smile

Presumably if we want two transferred we tell them when we go in for the transfer, or should I tell them in advance?

Boris that’s fantastic! Are you prepared for twins if you transfer two good quality blasts? Grin

At my clinic if you’re below 37, you have good quality blasts and it’s your first transfer, if you want two day 5 embryos transferred, you have to sign a waiver saying you understand the risks of twins and are doing so against medical advice. The likelihood of pregnancy counterintuitively doesn’t go up by much between 1 blast and 2 blasts, but the risk of twins if you do get pregnant is something like 50%. I think you’ll have a great bunch to choose from, so excited for you

One beautiful jelly bean seen this afternoon, measuring bang on for 8+5 and with an absolutely stonking little heartrate. Even got some little videos of the scan, just breathtaking. Could see a discernible head, and limb stumps, and where the umbilical cord was (just). Hang on tight there little duracell!

Boris, I'm with banana on the twins. If you have 2 good blasts transferred the likelihood of twins are pretty high! As banana says putting two back doesn't increase the chances of pregnancy by that much compared to one but increases the chances of twins by a lot. We are going for one at a time now as well.

I think I accept the risk of twins, we have fraternal twins in my family anyhow, it's always been quite a big risk for me. Although DH and I have just bought a bloody expensive car which, whilst better than our current sports car for babies, is not particularly sensible for twins. DH really wants twins...

Yes the eSET one at a time stuff UK clinics have to send you is very compelling

Before starting IVF I thought about twins as a BOGOF two for one special, but having read all the significantly increased morbidities associated with twins, and thought about the prospect of having to go on mat leave at 5 months / have babies in the NICU for several weeks, it definitely made me reassess

Same here banana, we actually transferred 2 blasts on our first IVF but bfn but I think there were other factors involved there which is part of the reason I'm doing fets instead. It has given me time to think though and one of my best friends has twins through IVF and they both have quite serious health problems and now they are toddlers she struggles to do much at all on her own with them. I would rather devote my attention to one baby at a time and I'm not sure I could deal with a long stay in NICU. It's a personal decision though so nothing wrong with wanting two Boris and lots of people have healthy twins.

Thanks guys for the food for thought. I'm hoping to have a chance to talk with DH about our options tonight for embryo transfers, once DSC are back with their DM. It's inpossible to FaceTime serious stuff at the moment. I've also asked my best mate, a GP for advice I trust her.

Just to add another spin on it Boris, with my immunes the chances are no matter how many transfers/embryos the chances are that nothing would stick without suppressent treatment. If we'd had 2 put back each time we'd be kicking ourselves now. It is a tough decision, hopefully speaking to your DH and friend has helped.

Again I'm like that Trixie as we put both of our only two back on our first try when I really believe we would have done better with a fet so I wish we had transferred one and saved one.

My consultant said similarly - he said the morbidities associated with twins posed an unacceptable risk when it came to DET with blasts (although we would have done two day 3-ers). He also said in cases of failed implantation of a good quality blast he may treat empirically with steroids & intralipids (depending on individual circumstances) for a subsequent transfer. So he didn't recommend putting all your eggs in one basket both in case it did work, and in case it didn't xx

Yes really wish I had done that on my first attempt but we were so excited about it all and thought if we put two back then we would have a really good chance of getting something. Little did I know....Anyway onwards and upwards. I would as you say transfer 2x3 day embryos which is what I think may happen this time around.

Does anyone's clinic test progesterone levels before trigger here to make sure they haven't peaked too early?

My clinic tested progesterone levels a few days before transfer but it was to make sure they were high enough.

Ah yes I can understand a few days before transfer Trixie but I am interested in a couple of days before collection to check they are not too high and the lining has not already started being receptive too early. I think I will probably book in for a private blood test the day before trigger just to double check as it's something that's been on my mind a lot. A lot of people with low reserve like myself suffer from premature lutenisation and once the follicles get to a certain size they start secreting progesterone too early which is bad news for lining receptivity. I think if I'm having a fresh cycle it would put my mind at rest and if it was high then I could show the NHS clinic and maybe they would allow me to do a fet later.

I didn't have it tested lucie (I didn't actually have a single blood test during either cycle) but if it were a concern then I would definitely get a private test too for reassurance xx

I'd definitely do it if it will put your mind at rest Lucie. It reminds me of the ERA test my clinic suggested I have where they do a kind of practice cycle and take a sample of the lining at the time of transfer to make sure it's receptive.

Yes I think I will book it in for Friday otherwise I will only be wondering. If the levels are low then i will know that I don't actually have the problem I've always suspected. Just they are quite nosey at my private clinic and most of them know me there now and I've already told the one nurse I'm having a fet but I suppose it could be that they want to test the progesterone before transfer like you had Trixie?! I will think of something lol.
