I saw my official fertility doctor for the first time on Tuesday last week (instead of my OBGyn. They ordered tests for rubella immunity, hbA1c and progesterone (CD27). Based on previous test results for fasting insulin she determined I'm slightly insulin resistant so put me on Metformin. I started with 500mg on Wednesday and I go up to 1000mg this coming Wednesday and then up again to 1500mg the Wednesday following and continuing at that amount. I have a follow-up in June.
The metformin triggered AF a day or so early so it was perfect timed to start this new cycle.
The doctor also gave me the forms for a HSG ultrasound and DH's semenalysis. We're holding off on those until April or May though to give the metformin time to work in case we don't need those tests (and thus waste that money).
I received the results of the blood work and I'm immune to rubella, hbA1c was 5.5 and progesterone was 5ng/mL (15nmol/L in the UK). I was just over their threshold for having had ovulated.
On talking to Purdie though, 15 is low for UK numbers so I did some research into the progesterone levels and found this article: www.scielo.br/scielo.php?script=sci_arttext&pid=S0100-879X2004001100007 that says:
"The study further demonstrated a significant enhancement in luteal progesterone concentration... in PCOS women treated with metformin. The results suggest that hyperinsulinemia/insulin resistance may be responsible for low progesterone levels during the luteal phase in PCOS. The luteal progesterone level may be enhanced in PCOS by decreasing insulin secretion with metformin."
Basically, they found that insulin resistance and low progesterone levels go hand in hand and that metformin helped raise progesterone levels by lowering insulin resistance. Which explains why they're not worried about treating the progesterone levels yet because they want to see what the metformin does for me.
I've read some positive stuff about metformin and I'm on a low(ish) carb diet. I haven't made an total switch but I'm trying to make better carb choices. It's hard on the weekend as work is far more structured but I'm trying! It's been 3 days so I'm cutting myself a little slack :)
