Looking for some advice.
My daughter is 3 in April.
She hardly eats but her weight is rapidly increasing. She doesn't snack she eats minimal at meals. She doesn't like sweet things she will eat fruit if anything.
She currently weights 31.8kg (5 stone) shes put on 14kg (2.2 stone) in a year.
Shes seeing a paediatrician and a paediatric endocrine specialists but yet we still have no answers.
I feel like im at a loose end.
Has anyone got any ideas?

Have they considered a pituitary issue? Benign pituitary tumours are actually common (I have one) and they can cause havoc with hormones and I know control weight and height etc. depending on where they are affects what the issues are but rapidly increasing weight is a common affect.

I dont think they have to be honest. We have had a GTT lots of blood a chest xray and a abdominal ultrasound

Shes nearly a meter tall. They have ruled that out as her cortisol is fine and she isn't short for her age

Has she had 9am cortisol and 24 hour urine collection for cortisol as that's the first thing I would push for. Cushings can't be ruled out with a random cortisol test.

She has had a few some before 9am with fasting and some after. She hadn't had a 24 hour urine collection for it though.

That’s not a reliable way to test for Cushing’s and there are also cyclical forms of Cushing’s which means the testings might have happened during a time when cortisol levels were low or normal. I would definitely push for further tests on that.

I will ask them to do that thank you. Would that show on the genetics panel?

I remember this from years ago. The boy was eventually diagnosed with Simpson-Golabi-Behmel syndrome. I know it’s rare, but his experience does seem similar to your daughter’s.

The 24 hour urine is quite reliable as its like an average of all cortisol excretion. But even then main reasons are benign pituitary/adrenal tumours. Which can sometimes only be picked up on MRI. Hopefully endocrinology will further investigate this. Don't think it shows on genetic testing so push for all options.
Sounds so difficult for you both. Her iron levels need looking at by what you have posted.

We have been waiting since December for iron and vitamin d from the endocrine team but they haven't done it yet

If you’re absolutely sure her diet isn’t related to her size then it could be several things: Cushings Syndrome, Hyperthyroidism, Prader Willi, GH Deficiency… your doctors will be able to say which more than people online.

That’s true, the pituitary controls many of the secondary hormone cascades in the body including the thyroid and therefore metabolism. See if they’ll do a brain MRI (with contrast if possible) to check.

Both hospital seem to be taking for everything this is why Im asking here. We are waiting on genetics blood test to come back.

Ive been keeping food diaries. Yesterday she had a handful of cereal (NO sugar) ,1/2 a small sausage roll, chicken burger with 1/2 the bun and 8 chips all day. She drinks water most of the time but does have sugar free juice sometimes. She will have milk at bedtime

When you say a handful, that can be a lot or a little depending on your definition, you’d be better off to weigh everything. I’m sure you could get some digital scales at a reasonable price online x

I don't know if this is much help, but I noticed her iron and ferritin are on the lower side. I gained a stone in a short amount of time when mine was low. What's her energy levels like compared to a year ago?

That does sound worrying. Her food intake isn’t high from your example but the type of food isn’t good, though am assuming this is just a one off rather than her normal diet. What are her exercise levels like? Does she run around a lot and do activities?

That's 1 day u have read. She eats the same food but less as my older children and doesn't have the sweet things that my older children have. She would prefer to eat a piece of fruit over anything so far for junk. Judging much!!

Keep pushing every health professional. Surprised though that you’ve had her ears pierced with jewellery like that at this age.
Have they done tests? What results have come back?

I’m equally surprised that you still don’t know how to conduct yourself at your age. Shame.

She gets breathless and wheezy quite alot. There's quite a few things shes being tested for but we just keep getting wait for genetic
