Can i have an urgent opinion please?? **Trigger Warning - Graphic Images**

My dc is under paed care (I have another thread about auto immune) basically he’s had explosive diarrhoea, mucousy stools and stomach ache since January. He’s had two lot of stool samples and all fine? Had bloods and all ok apart from Ana abnormal and low ferritin. He had those bloods re done last week to check. Paed said primal food intolerance, said 4 weeks dairy free, 4 weeks wheat free (currently half way through) and 4 weeks low fructose and ‘see you in a year’ Hmm

Anyway about 2 weeks ago he starts having blood in his poo/the water. GP not concerned and said probably piles. Today he’s told me he’s being feeling something ‘there’ in his backside, been feeling it a while, didn’t want me to look so I said probably piles don’t worry and if he would feel happy me checking then to let me know. He’s just called me and said it’s happening and let me look. It doesn’t look like piles, I’m petrified it’s some sort of prolapse? He said it happens when he wees, poos and sometimes randomly. I could see whatever it is sort of come out his back side as he was on all fours...... he said when it happened is when the mucous comes out. This is all new to me and I’m really worried. Do I need to take him to gp tonight?? What does a prolapse look like? Could it really be just piles? He isn’t itchy ...

Sorry your dc is unwell - how old is he? If under 3 I’d probably go to ooh walk in if over I’d call the gp and see what they say.

Sorry that’s important info! He’s 13. He’s Also autistic which hasn’t helped this process.

It could well be piles, yes. I would go back to Gp and ask them to look at it with you to reassure you.

Poor lad. You sound concerned. I would take him to the hospital or gp if it's there now. You can call 111 for their advice.

Given his age I think I’d call your gp - they will have a link to ooh service and then the gp you chat to can decide whether it will wait until Monday. 111 may be helpful but I’d do gp first then you can tell the triage person ds is autistic and may not cope with the experience. 111 may just say a and e.

There’s just 111 and whenever Iv used them in the past they take about 4 hours to call back (today being Friday may well be super busy too) I don’t want to drag him unnecessarily out tonight if this is ok for me to wait to try to see someone tomorrow?

It doesn’t look like piles.... Iv never seen anything like it.... without meaning to be crude but it almost looks like a pair of labia or something coming out.... shiny looking and very dark in colour, dark red. It’s hard to explain and don’t want to post a picture on here. Is there anyway to tell the difference?

Will it wait if I call 111 first thing in the morning?

GP only says to call 111.... or 999 for emergency. Can’t speak to a GP.

Could be piles - you can have internal or external haemorrhoids. Fresh blood might be a sign of piles, blood from further back in the digestive tract is likely to be brown or even black. Probably worth a chat with your gp next week or ooh if you think urgent

Piles can be very dark red, especially if they’re internal and have come out of the anus but partially occluded by the anal muscles. It stops the blood flow to / out of them and makes them look like a deep bruise colour if that makes sense.

Obviously if you’re worried then 111 is your point of call.

I would tell 111 and ask for a gp call. It could be painful and he might be scared to go to the loo which makes things worse.

I can’t take him to a&e in a Friday night, he would not cope with that at all (I’m also a LP with younger dc who would have to come) of course I would if it was an accident ....

He must’ve been like this a while. Earlier this year he said his back side was itchy when this started and I gave him cream and it seemed to stop. Then he had been fiddling with his back side when opening his bowels saying it was ‘annoying’ but very vague and wouldn’t let me have a look. The last week he has been sitting on the toilet and leaning right over to go. I told him to stop doing it. He isn’t constipated, he has bad diarrhoea.... paediatrician isn’t remotely interested. GP has been ok and kept pushing the paediatrician but his follow up is next July!!! I have a telephone appointment to discuss the Ana blood results (they took 3 weeks last time) but that’s next Wednesday.

Could this all be linked? Or just a problem resulting the diarrhoea?

He isn’t scared to go to the toilet thankfully and hasn’t said it hurts, just his stomach hurts.

He should see a doctor maybe get a colonoscopy. They could be piles or polyps. Has he been tested for giardiasis?

Why wouldn’t he cope with a&e on a Friday night?
If you’re worried it’s serious OP you should take him to a&e regardless tbh
It sounds like it could be piles but really hard to say, he needs to be properly examined and give the hcp the full details

His symptoms sound a lot like Giardasis...did they check for that?
Giardiasis

Giardiasis is a tummy bug that causes symptoms like diarrhoea, farting and bloating. It usually goes away in about a week if it's treated, but can sometimes last much longer.

How giardiasis is spread

There are lots of ways you can catch giardiasis, such as:

drinking water that's not been treated to kill germs (usually while travelling in developing countries)
water getting in your mouth while swimming in places like lakes, rivers or swimming pools
eating food that's been washed in untreated water or handled by someone with the infection
touching surfaces that have been touched by an infected person
having sex – especially unprotected anal sex
You can become infected if small bits of poo from an infected person get in your mouth.

Symptoms of giardiasis

The main symptoms of giardiasis are:

smelly diarrhoea
tummy pain or cramps
farting (flatulence)
smelly burps – they may smell like eggs
bloating
weight loss
You can also have it and be able to spread it to others without having any symptoms.

Non-urgent advice: Call your GP surgery or 111 if:
you have had diarrhoea for more than a week
you have bloody diarrhoea or bleeding from your bottom
It's best to call rather than visit a GP surgery as you might have an infection that can spread easily to others.

Tell the GP if you have recently travelled abroad.

All Iv been told is his stool tests were all fine, no infection. His blood tests were also fine except his ANA was abnormal, low ferritin and smaller than average red blood cells...... Iv spoken to two paediatricians and they aren’t bothered!! Iv been having to do this silly diets with him as they are adamant it’s food intolerance but there’s been no change at all on trying it.

His full blood counts shows no infections apparantly. He was tested via blood for coeliacs but the coeliacs society said you can get false negative (gp agreed and gp said it can’t be ruled out at the moment)

I read your previous thread. Could you take him to A&E or a walk in centre tomorrow morning when it'll be quieter?

And he wouldn’t cope with a&e on a Friday night because he is currently getting ready for bed has already had his prescribed melatonin and he’s autistic and struggles with a normal day let alone a random completely scary night time trip to a busy a&e department.
