Can i have an urgent opinion please?? **Trigger Warning - Graphic Images**

My dc is under paed care (I have another thread about auto immune) basically he’s had explosive diarrhoea, mucousy stools and stomach ache since January. He’s had two lot of stool samples and all fine? Had bloods and all ok apart from Ana abnormal and low ferritin. He had those bloods re done last week to check. Paed said primal food intolerance, said 4 weeks dairy free, 4 weeks wheat free (currently half way through) and 4 weeks low fructose and ‘see you in a year’ Hmm

Anyway about 2 weeks ago he starts having blood in his poo/the water. GP not concerned and said probably piles. Today he’s told me he’s being feeling something ‘there’ in his backside, been feeling it a while, didn’t want me to look so I said probably piles don’t worry and if he would feel happy me checking then to let me know. He’s just called me and said it’s happening and let me look. It doesn’t look like piles, I’m petrified it’s some sort of prolapse? He said it happens when he wees, poos and sometimes randomly. I could see whatever it is sort of come out his back side as he was on all fours...... he said when it happened is when the mucous comes out. This is all new to me and I’m really worried. Do I need to take him to gp tonight?? What does a prolapse look like? Could it really be just piles? He isn’t itchy ...

Hi I usually have only seen this in Alderney person and they get a pessary sort of like a ring to hold it back up , it definitely needs medical attention I would take him to a&e as need it dealt with ASAP, if you phone 111 they will prob give you an appointment at a walk in centre which means he'll be seen quicker. Poor boy it must feel awful

He’s actually ok, hasn’t complained at all and sitting down fine. So it clearly isn’t ‘out’ at the moment? I’m waiting for a call back for an appointment, out of hours at a hospital. They have until 12:30 apparabtly 111 said when I rang back to chase.

It could be a skin tag which is protruding out of his anus, they can feel like something is coming out of your bottom, they can cause pain when protruding and they can also bleed after a bowl movement.

Also, when did he start taking melatonin? Was this more than 9 months ago?

It’s one hell of a skin tag if that’s the case, it’s huge!

No he was put on that this year I think. So this all started in January, he was out on melatonin maybe Easter/May possibly.

Is he taking other medication than the melatonin? I’m just wondering if the loose stools could be a side effect of medication?

Also anxiety causes IBS, I know that the pead said he doesn’t have it but anxiety is related to autism so it could be IBS.

He’s asthmatic so has steroids which he’s been on for many years probably 10 years. Then he’s on fluoxetine which he’s been on a couple of years.

I have a large skin tag it gets painful and bleeds, it pops in and out...

No he’s no intervention, that’s what I’m tryinf to push for but the paed thinks unnecessary. Hence my earlier question about what I should ‘reasonably’ expect from the appointment today? I feel like kicking off to be honest. Not in a rude, aggressive manner as that is not me but something has to be done....

If he’s had no investigations only blood tests I’d ask for a colonoscopy. If you’re getting no where with the pead you can speak to PALS at your hospital and can ask for a second opinion with another pead or at a different hospital.

He paed had a visual look but not internally no.

The hospital he is under is amazing but the particular paed he’s under is shit to be quite frank. Only looked up at us once during the apt and just wasn’t listening and had his mind set on it being an intolerance (he said milk) but we’ve been doing the exclusions and no change at all.

He’s just been to the toilet and I asked him how it was and he said hard to pass so it wasn’t diarrhoea.... he’s not had that before....

It might just be a normal bowl movement. Does he get normal ones in between having loose stools?

Does he suffer from anxiety? Do you keep a food diary, if so does he have loose stools after eating certain foods or experiencing anxiety?

Been keeping diary and nothing stands out. He says nothing particular makes it happen. No until now he’s been going 4-8 times a day with diarrhoea. Just had call and taking him to see gp.

So it is a prolapse but a small one and it can wait apparently. She said isn’t emergency and that I should go to gp.

Low ferritin is a marker for coeliac

Get the paed to do genetic test for HLA markets for coeliac and then of positive they can do duodenal biopsies to see if that's what's causing the diarrhoea
You can stop the lactose now but don't cut out gluten until after the test
