I have cousins with surnames starting with D, one has first initial V no middle name other has first and middle M A
Uncle's initial are D A D (not unfortunate)

Old school friend was Sarah-Louise middle initial A surname started with a G (poor girl)

I knew someone who admitted to accidentally giving her daughter the initials MAD. The first name was a combination of her and her mother's name, so they gave MIL's name as a middle name, and never thought it through.

Just remembered another girl I went to college with who had the initials IRA, an American girl with an initial necklace who had no idea why all the locals were looking at her necklace so weirdly

my youngest has initials BM
including middle name BSM
no clue if anyone other than me has noticed, i've never commented on it.
btw he's #3 and dh picked the name. with 3 under three i had zero name inspiration and voted for daniel or danielle

A friend's father's first names were Vernon Douglas. Of course, there was also Val Doonican (Google him if you really want to know, whippersnappers).
The worst though was a work colleague who was obviously named before the word his initials and last name formed became a rude word.
Too outing (for him) to put it here, but exactly the same effect as Mr & Mrs Serr calling their son Thomas Oliver Simon.

I used to work with someone who gave her DD the initials BO. The poor child also has a dreadful first name that is a frequently mispronounced boy's name. Neither mum or dad are known for their sense or intelligence so it's to be expected I suppose! They have an older DS with the most classic and normal boy's name imaginable so why they felt the need to saddle their poor DD with such an awful name I don't know!
