DD vomiting bug - A&E?

DD (19) has had really severe diarrhoea and vomiting for the past 24 hours. Can’t keep anything down whatsoever, at this point I don’t know how she even has any fluid in her tummy to vomit - we’ve tried small sips of electrolytes but 10 mins later they come straight back up again. She’s feeling awful - really weak + dehydrated.

Also has a very high temp of 39.5 degrees, again she can’t keep any paracetamol/ibuprofen down to make her more comfortable

AIBU to take her to A&E? Please be kind - I’ve never seen her so ill before and don’t know what to do. I’m concerned that she’s going to end up needing IV fluids as she isn’t peeing and she’s desperately thirsty

You should never reheat rice. It has been known to kill you.

You can reheat rice as long as it's piping hot and has been cooled quickly and refrigerated.

The main benefit of checking OBS in this instance is that if they're deranged than it can be the last push to definitely go.

It's really not common that D&V 24hr history of, in an otherwise healthy adult would result in a need for treatment - hence the hunting for evidence of a benefit to going to a&e.

She hasnt peed for 15 hours so i think that warrants a trip by itself. Regardless of anything else

My point was. I have had kidney injury many times and my obs were ok every time

So ‘if’ they were ok, its not a reason to stay home
….
Not peeing and a Temperature that isnt going down are also two signs of Sepsis !!!

I would take her to A&E if I was you... Better safe than sorry.

I wouldn't at 24 hrs, I'd be trying to see a GP for some nausea meds, and that would be it. slightly elevated HR and BP- would usually mean not clinically dehydrated- BP would be low. She's 19, and with no other medical history, at 24 hrs it isn't that long- but definitely should be trying to control the nausea if you can get an emergency GP appt- realistically- she'll sit in a WR for hours vomiting and using public toilets for the diarrhoea- she won't be taken in quickly.

She's young, she's compensating for now but she can't keep fluid down, has a serious temp and hasn't weed for 14 hours. This is not good. You need to take her in. There is some seriously dangerous advice on this thread. She needs prompt evaluation.

Rice is one of the worst things for keeping. If you have any to spare, chill it and freeze it ASAP. If it's been hanging about, bin it. Of course, this advice is too late for your DD who is suffering the consequences but I imagine she won't do that again!

Electrolytes are important. Try her with Diarolyte - make the mixture and freeze it into ice cubes - you've got to keep those kidneys working. If her pee is very dark, she needs liquids.

There is no pee! The icu cube point was hours ago. The poor kid needs evaluation and probably ivs. Which will likely be a bugger to do as she's so dehydrated.

A&E nurse here. Please take her in. I know the thought of an A&E waiting room isn't pleasant when she's feeling like this but not urinating for that long and the temperature are real red flags and she needs treatment. Just because she is young doesn't mean she won't have an acute kidney injury from being dehydrated and with her temp, slightly elevated obs and not urinating I think she would trigger the sepsis screen.

Update: temperature has gone down slightly and sickness seems to have subsided after giving her tummy a few hours to rest from drinking. Also passed urine. She’s been able to suck some antisickness tablets and so far so good, they’ve stayed down. Will give them a bit of time to kick in and then have her try some small sips of fluids again. Will hopefully be able to get some ibuprofen in to her to take the fever down a bit more.

We all had this last week. I experienced everything similar to your daughter (I’m in my 40s though). I couldn’t eat or sip water (could barely leave the bathroom for hours). I got the chills too.

The worst of it was over in 48 hours, while I couldn’t keep it all down, I found I was improving. Five days later it was gone. If it’s what is flying around, it is short-lived but really awful.

Can you make a telephone consultation initially and take it from there?

That is simple NOT TRUE !

Iv had kidney injury because of dehydration and my blood pressure was fine

I'm looking after a 20 year old DS at the moment who's running a temperature and under the weather, chest infection I suspect, is super stressful, they are just our very large babies!!

I'm glad to hear she seems to be on the other side.

Yes - have plenty in the house (thankfully!), it seems to have settled her tummy enough to have a much needed glass of orange squash.

Yes! Looks likely I’ll have to go in to work tomorrow though - one of our team is already off sick so I really can’t afford to take it off. I’m only 2 mins down the road so at least if DD needs me she can ring

Food poisoning doesn't typically cause a fever.

Theres good advice surrounding management of symptoms here:

They outline some red flags to watch for which if you see highlight could be an emergency.

Commonly otherwise healthy young adults wouldn't normally need to be seen unless diarrhoea over 7 days or vomiting more than 2 days.

If she's not passed any urine around/over 18 hours then this is concerning/need a review. The difficulty with 111 is they can't see her and follow an algorithm flow chart on what advice to give you - if she can't/or you say she can't drink any fluid at all or she can't pass urine they'll tell you to goto A&E.

Doesn't mean it doesn't feel awful, but if she can keep fluid down enough to absorb some (about 15min), and is still peeing then I would try to avoid a&e unless you think it's really an emergency.

Food poisoning can cause a fever. When I've had food poisoning in the past always had a fever with it
