DD vomiting bug - A&E?

DD (19) has had really severe diarrhoea and vomiting for the past 24 hours. Can’t keep anything down whatsoever, at this point I don’t know how she even has any fluid in her tummy to vomit - we’ve tried small sips of electrolytes but 10 mins later they come straight back up again. She’s feeling awful - really weak + dehydrated.

Also has a very high temp of 39.5 degrees, again she can’t keep any paracetamol/ibuprofen down to make her more comfortable

AIBU to take her to A&E? Please be kind - I’ve never seen her so ill before and don’t know what to do. I’m concerned that she’s going to end up needing IV fluids as she isn’t peeing and she’s desperately thirsty

Omg she has my sympathy if she has food poisoning from rice, I had it at a similar age and can still remember how awful it is.

She needs to have a teaspoon of water/squash/rehydration fluids every 5-10 minutes. Don’t have anymore or she’ll throw it straight back up.

Yes I would go to a&e, it should be getting better by 24hrs, my son had sepsis snd the symptoms were just like a vomiting bug if I had left it longer, I dread to think how it would of turned out.

Couldn't keep fluids down
High temp
Legs hurt/difficulty in walking
Check her stomach/back for a rash

Food poisoning doesn't typically cause a fever.

Theres good advice surrounding management of symptoms here:

They outline some red flags to watch for which if you see highlight could be an emergency.

Commonly otherwise healthy young adults wouldn't normally need to be seen unless diarrhoea over 7 days or vomiting more than 2 days.

If she's not passed any urine around/over 18 hours then this is concerning/need a review. The difficulty with 111 is they can't see her and follow an algorithm flow chart on what advice to give you - if she can't/or you say she can't drink any fluid at all or she can't pass urine they'll tell you to goto A&E.

Doesn't mean it doesn't feel awful, but if she can keep fluid down enough to absorb some (about 15min), and is still peeing then I would try to avoid a&e unless you think it's really an emergency.

Yes, that's what I think. It's all very well suggesting sips of fluid but that's not helpful when someone can't keep anything down.

Agree with other posters here, it sounds like food poisoning or norovirus, and if I had to give a guess, it will be the former, I would keep going with the sips of water or whatever she is more amenable to. If you go to the hospital you will potentially be in the same position but more awful in respect of vomiting and having the *hits in public. Sleep. I got food poisoning this year in Ankara and had agonising pains (like flu) and although did not vomit I was up every half an hour to go the loo, other than that I was mainly unconscious. I drank tons of water. I lost 2 days. My 18 year old daughter sat at my bedside and was really frightened for me. I recovered eventually, but it is scary for those involved. xx

Definitely get a medical opinion, eg call 111 now. The temperature sounds concerning.

Really weird advice on here. I’ve never known food poisoning to give such a high temp. I’d call 111 for proper advice. Does she have any signs of appendicitis other than sickness and high temp?

Years ago I was similar and it was noro. Dh was so worried he called 111 and they sent a gp to our house middle of the night. I was only 28 and he said I was severely dehydrated and if I was sick again within 4 hours I’d need to be in hospital. He gave me an anti sickness injection. Thankfully it worked. The blasé attitude on here is quite dangerous.

Contact 111, but be prepared for a long wait. Try a warm water bottle on the tum, and go to the pharmacy for buccastem anti-sickness tabs in the morning, they sit under the lip and absorbed by the soft tissues. I'd personally see how she fares resting her stomach for an hour or two before trying to sip electrolytes again.

Too right on the long wait!! I was on today and call backs were approaching 14 hours. I booked an ambulance for someone who hit their head couldn't get up (elderly) this morning, they still were on the floor when I went home at 10 tonight :(

Do less than a sip. Do a teaspoon every 10 mins and see if she can keep it down. She should absorb that quickly as it's such minute amounts and she's already dry.
Is she hasn't weed for more than 12 hours or so I think you might need have her seen by hospital especially as you can't keep the temp down either. Hope she's better soon.

Remember most people on Mumsnet say they wouldn't go to A&E even if one of their legs was falling off. Weird NHS virtue signalling. Just get her seen by professionals, better to be safe than sorry.

I wouldn't rule it out. I've been to A&E with severe vomiting before. People might not think it's that serious but that's a very high temperature and she might need fluids or IV paracetamol.

Your kidneys can start failing if you become very dehydrated so it's worth calling 111 and see what they say.

I was going to say the same, My DD was on the verge of her kidneys failing, she was severely dehydrated from a vomiting bug, she was kept in on a drip overnight and then monitored.

It sounds like she needs to be on some fluids. I'd also be concerned about an infection with that high temperature.

I'd ring 111 and get some advice. Make sure you mention the fact that she isn't urinating.

NHS states that food poisoning can cause fevers of 38degrees plus. I don’t think it’s blasé to see if a self limiting illness is just that. If I had food poisoning the last place I’d want to be is in rbe waiting room of an A&E unless I desperately needed to be there. If want to be at home in my own bed, sipping fluids waiting for it to pass.

I think it sounds like she could be getting dangerously dehydrated. Calling 111 could take hours. I'd do the online 111 thing and see what that says. I think she probably needs to be seen but if you can say you've been told that by 111 they tend to take it more seriously.

Rice is dreadful for this as it grows bacteria like mad. Just to spread awareness, any leftover rice must be refrigerated promptly and not allowed to sit out. If it does sit out, dispose of it. If properly stored it is good for 48 hours in the refrigerator. After that, bin it.

I would say go with your instincts.

D & V can be serious. How long has she been without peeing? That can be a sign of severe infection, so important to know how long it's been.

At the very least I would get advice from 111. Lack of electrolytes can bring about problems very quickly. While this sort of thing usually resolves, sometimes a person needs medical help. Go with your instincts and don't feel bad. I wish her well x

You can’t demand anything .
@HighlandCow78 try 111 first to see what they say , personally I’d sit it out a bit if it were me as the last place I’d want to be with D&V is an A&E .

I don’t think you’re being unreasonable and I’m a nurse! I had to go myself a few months ago as I was so weak, vomiting and diarrhoea constantly at the same time and wasn’t keeping anything at all down. They did bloods there and my kidney bloods were very dry and I needed 3 bags of IV hartmanns which is fluid with electrolytes and also iv antisickness

That’s the worst! I hope she starts to recover soon. If things don’t improve dial 111, they’ll get her an out of hours appointment and if a&E is needed you’ll at least be told a slot to go in for. Instead of waiting hours.

I’ve had two bouts of food poisoning. The first I was admitted for two nights on a drip for multiple bags of fluid. The second time I passed out in my shower and smashed through the screen unconscious. Best to be safe than sorry.

I was thinking more along the lines of food poisoning? She ate some lukewarm reheated rice yesterday - teenagers!! 😩

You don't need to have met someone with it, its air carried and you can also pick it up off surfaces.

Food poisoning doesnt bring a fever usually. Noro however...

but if not weeing in 24hrs then deffo 111 for advice.

It's shit, no pun intended, but proper Noro isnt just the squits and a few voms. It's a hideous multi day, staying on the bathroom floor situation for many.

She needs to keep drinking fluids even if they arent mostly staying down. It can help flush the virus quicker too.

THIS!!! Please, please check with 111 as its so much better to be safe than sorry when considering if it could be sepsis. Hopefully it's just a bad bug or food poisoning but with these symptoms I would always want to rule out sepsis (have worked clinically and attended sepsis training many times in previous role, so it's always in the back of my mind when someone is poorly). Symptoms can include lack of urinary output, vomiting, diarrhoea, fever, rash, changes in breathing etc. I don't want to scare you but also couldn't scroll and not comment just in case xxx hope she is all better very soon! Xxx

No signs of appendicitis. I’ve given 111 a call but typically where we are it’s good few hours before they call back (if it all sometimes😬). I’ve decided that if by morning she still can’t keep anything down, hasn’t peed and still has the temp then we’ll head to A&E. Don’t think there’s anything else that we can do!
