It wasn't that she wasn't physically capable of calling a doctor, it's that she was under intense pressure not to, because of the circumstances she was in at the time. Circumstances that didn't exist before she was married to a prince, or when she was dealing with issues other than an impending mental breakdown, which gets a different reaction from people.
In that sense, the Firm was actually probably right. The amount of hate she gets merely for not being happy, as per this delightful post, is unwarranted. The amount she would get if it got out that she is so unhappy that she requires professional mental health care...well, once again, we are seeing it right now, aren't we?
I can certainly see why she wouldn't have called a doctor if she would have got responses like this post (which, to be fair, is only one of many) and had the entire Palace PR team telling her she couldn't. Can't you?
