It's way to easy to write Trump off as being mentally unwell. He almost certainly has an personality disorder but that does not mean he is mentally unwell.
He is just a bad man. It's ok to say that, he's a spoilt, mean nasty man who was never told no as a child.
But personality disorders are defined, in fact, as mental illnesses. eg:
Personality disorders are a group of mental illnesses. They involve long-term patterns of thoughts and behaviours that are unhealthy and inflexible.
According to the ICD and DSM, both ways of classifying psychiatric conditions, personality disorders are considered to be mental disorders, and therefore mental illness.
Although onlookers and insiders can assume Trump has NPD or ASPD, we cannot make a definitive medical assessment. But colloquially speaking, he is clearly a narcissist and/or a sociopath, his ego is overblown and easily punctured, he is grandiose and exceedingly low if not out right lacking in empathy, and so on. He is not a well man!
Jails are full of sociopaths and narcissists, though, and it is not considered the sort of mental illness that would let Trump off the hook for his actions.
