A bizarre ramble amid other bizarre rambles. But actually he tells us which bit is important to him by saying it twice: "wouldn't have shown the pain".
Why was that idea bobbing near the top of his tiny, sewage-choked brain?
Suggests to me he's undergoing a lot events at the moment during which he aspires not to show the pain and to act like nothing happened. You could even add, that he has to keep going back despite the pain.
Frequent minor medical procedures would fit the bill nicely. A cannula in the back of the hand, for example...
