Can you help me read this gp note see pic

I have been trying to find out why I went to SEN school so asked for my GP notes incase there was anything cant find anything but coming across other stuff. The notes in the pic seem to be from 1977 -80s can anyone read them ? Thank you

All the entries seem to be abbreviated notes of the various medicines that were prescribed for you, and how often you were to take them.
For example the first line starts
Pen V Elix
ie Penicillin Elixir, an antibiotic liquid medicine.

There doesn't seem to be any mention of what ailments you were suffering from to require the medication.

I remember your first thread about your quest, so I hope you feel you've made some progress in your search for clarification.

That writing is atrocious. The only thing I can read is 'ear infection '.

You probably need someone with a medical background. Maybe post in one of the health topics if you don't have much luck here, op.

Think the 3rd line might be Phenergan? Elixir 5mls
4th line ? Syrup
phenergan used to be prescribed to help babies sleep

it looks like a summary of meds taken. The dates are all written in the same handwriting same pen and getting progressively faster. I know documentation has gotten more important the last 50years but can't imagine it was this bad....maybe it was.
Perhaps I'm using current experiences but would you be seeing the same doctor who uses the same pen over 4 years

@Wishiwasatailor Maybe the doctor was using a fountain pen? Far more people used them 50 years ago. My husband has used the same fountain pen for the last 30 years.

No nothing at all. I asked from birth the only thing it shows from close to birth is the pic that I posted . Then it skips to mid 90s . There's loads of stuff missing in general. Like I have 6 children but it only mentions 1 child birth none of the others .

Its basically missing all of my childhood records.

Lots of stuff thete that i had blocked out
Nothing helpful that i needed though.

Dates are years 77 and 78 in the notes. So at least robots aren’t taking over soon 😂

Every truism ever about GPs’ poor handwriting! That’s shocking. I got ear infection, some dosages and that’s it. I can’t see anything about SEN - but who would know.

Makes you wonder whether GPs were taught to write illegibly in case they got sued…!

Have you tried the nhs app? Mine has a record of a broken arm in 1988.

Its just recent stuff . That's why I made a request for all of my history. But there's alot missing i have emailed to ask about it .

If you live in England, so does yours. The might be in paper form or digitalised, but they do exist.

OP, were you in England during the missing years? If so, the records will be somewhere. If you were with a practice who digitalised their records early on they might even by on a floppy disk or CD but this should be kept with your notes.

Yes I was . Bit disappointed to be honest. I knew there was a possibility I would not find what i wanted. But disappointed that the whole of my childhood notes are missing. There's alot of my adult ones missing to. Although im not so bothered about them still majew me think what else is missing
