Can you help me read this gp note see pic

I have been trying to find out why I went to SEN school so asked for my GP notes incase there was anything cant find anything but coming across other stuff. The notes in the pic seem to be from 1977 -80s can anyone read them ? Thank you

Yes I was . Bit disappointed to be honest. I knew there was a possibility I would not find what i wanted. But disappointed that the whole of my childhood notes are missing. There's alot of my adult ones missing to. Although im not so bothered about them still majew me think what else is missing

Yeah, it’s disappointing but also fairly important that your records are found if you think there could be a significant diagnosis there. Your GP surgery will know how to try & track them down. Best of luck.

I mean they're the ones who haven't properly stored them and provided them to the OP? Why would they suddenly magic them up

It seems odd to me that massive chunks are missing. They are missing the birth of 5 children. And God knows what else. Im not really bothered about the kids birth although nice to reflect on.

The most important is from age 7 to probably 16. And im missing everything from that so not happy at all.

I did email them but I think I was a bit jumbled. So I may message them again. If I tell them I need age 7-16 maybe its somewhere dont know. Can only ask

It’s not necessarily them at fault unless the OP has been with the same practice all her life.

Records most commonly get lost in transit from one practice to another or are actually there but in a format nobody’s looking for (on a CD etc).

Only electronic records go directly from one practice to another, and only then if both practices use the same system. Everything else (commonly everything pre 2005 ish but can be later) is sent via the central FHSA & that’s where the surgery would need to start chasing.

It’s really quite common for bits to be missing and, unless the patient tells the doctor about a past problem that isn’t recorded, they wouldn’t always notice. It’s not that unusual for a healthy person to not visit a GP for 10 years, although a much longer gap should really raise suspicions.

Sorry - it’s the Family Health Services Authority. I would just say “if these are the only records you hold for me then quite a lot hasn’t been passed over from my previous practice, can you help track them down please?”

The Receptionist may not know exactly how to do that but somebody in the practice will.

This is what I can make out (and the ChatGPT interpretation is just rubbish, it says absolutely none of that!). I have printed it out and enlarged it and the only things I can read are nearly all medications, as follows.

29/4/78 Phenergan or Phenytoin
24/11/78 Phenergan or Phenytoin
29/11/78 Chloral paediatric
8/6/79 query ear infection

Are you sure? If the LA funded a special school there will often have been a social work assessment at some point. Not invariably, but it would be common.

Well, the medicines point to the need for sedation, so either sleep or behavioural problems probably.

I'm sure you have thought of this but please mention all previous surnames as your notes don't always get collated if you change it. And get your current practice to get previous practices to check under those names too as your Lloyd George's could possibly still be on their shelves.

If it does say Phenytoin rather than Phenergan, that is an anti-epileptic. But Phenergan seems more likely esp in view of the later prescription of Chloral.

What back in the 80s-90s? My ds had sen /ehcp but never a social worker. He had educational psychologist though. Maybe it was different then ?

if you went to ‘special care’ school it might be more worthwhile trying the old LA or school for the Sen report.

although it’s likely most of the information you’re looking for no longer exists

Why was i being sedated at 18 months old 😅. Was around 3 years the 2nd time.

My mum had 8 kids by then maybe she needed a break /me to sleep 😴
