Can you help me read this gp note see pic

I have been trying to find out why I went to SEN school so asked for my GP notes incase there was anything cant find anything but coming across other stuff. The notes in the pic seem to be from 1977 -80s can anyone read them ? Thank you

If you were 18 months old then the Phenergan (if it is that) and Chloral were most likely for sleep.

In the late 1970s my siblings & I had Phenergan for car sickness on long journeys. Could it be something as simple as that?

if you went to ‘special care’ school it might be more worthwhile trying the old LA or school for the Sen report.

although it’s likely most of the information you’re looking for no longer exists

I know it doesn't help me find why I went to sen school. But I just wanted to know what it's said. The dates are wrong anyway. I was 3 and under in them notes.

Its a possibility it doesn't exciist but it should do really. I asked the old school no Joy. I was just hoping there may be something in gp record. LA feels harder to do. But I will try though.

I assume you are currently registered with a GP practice and have made a formal subject access request (under GDPR) to them? If you are registered with a practice then they should hold your whole record including any paper part, although it may be stored offsite. The missing elements could be because they’ve withheld information as they are entitled to do so under GDPR although there are rules about it - the Information Commissioner’s Office website sets this out, or they’ve just gone missing when you’ve moved practice in the past. Unfortunately this does happen.

If you’ve not made a SAR, I’d be explicit to your practice you are making one.

If you’re not registered with a GP and are or were in England, I think records are held with Primary Care Support England. Please don’t ask about family health services authorities because they were abolished in the 1990s.

Im confused how did you find that, none of it is applicable to me . I was 3 and under for them things. I just wondering generally how you found it . Is it via a BOT ?

I don't think that version is right OP (surely a bot?), the first line is clearly Pen V elix 62.5 mg tds. theDudesmummy is closest to what it says I reckon.

22/11/77 says Penicillin V elixir and the dosage (62.5mg tds) not 6.5 years

29/4/78 I don't see "wet at night"? Possibly something about a reflex?

8/5/78 either syrup or symptoms, can't tell which. I don't see URTI?

29/11/78 says Chloral pediatric and the dosage (5ml but frequency not clear). I don't see pleurisy?

I could not see any entry for 1/5/79. Where are you seeing that? Is there another page or something? I can't see any entry about therapy or schoolwork.

24/11/79 says elixir Phenytoin or Phenergan and the dosage (5ml nocte). I can't see anything else on that date. I also don't see "slow progress" anywhere?

Your "8/11/80" would appear to refer to the "ear infection" entry which is on 8/6/79. It may say penicillin but doesn't really look like it. (I wondered whether it may say pyrexial but it doesn't really look that either, so I am stumped there).

Your 12/1/80 (could be 12 Jan, I had it as 17 jan, and it could be 80 or 81) says Chloral again and the dose (2.5 to 5ml, frequency not clear but probably nocte). I see nothing about pain.

Where are you seeing Quetiapine? To give an antipsychotic to an eighteen month old baby would be unusual to say the least.

It is risky and not kind to offer clearly wrong AI generated answers when someone is trying to find out about their medical history. It could have a real bearing on all sorts of things in their life. AI "hallucinates" all the time, with sometimes serious consequences.

If my dd had seen that it would have caused a big issue and and panic for her. Its only because the age didn't add up that I could see it wasn't right.

I asked gp reception about the rest of my records. She said this

The only other thing you can do is go on the website for NHS England and enquire to see if they have any more records for you.

It's not my interpretation - it was straight from ChatGPT. 😅 I'm supposed to be working too - hence the quick copy and paste. Part of my job entails supporting children to improve handwriting legibility... I can tell you this GP was not working at the expected standards 🤣
