Can you help me read this gp note see pic

I have been trying to find out why I went to SEN school so asked for my GP notes incase there was anything cant find anything but coming across other stuff. The notes in the pic seem to be from 1977 -80s can anyone read them ? Thank you

I think I'm probably wrong on that one, I only know that name because I got it as a medical student! Unlikely to be given to a young child. But might be worth googling the brand names of childhood vaccines in the 1970s. I used to be in a research role pulling vaccine data from old GP records and it was pretty pointless due to the poor legibility.

OP did you have a son in 1981? The last line looks a bit like 'son illegible'

The lines saying phenytoin are pretty clear so I'd be asking your relatives if they recall you being treated for seizures. Children can grow out of them so may not have continued into adulthood. Not sure if chloral hydrate was used for epilepsy in 1970s

Op said she was under 3 on all these dates written.

GP here, OP - unfortunately this sort of single line scrawl for each medical consultation was fairly standard up until computerisation. I agree with those who see Phenergan 5mg nocte, and the reference to an ear infection.

And even if you are not aware of having been seen by them per se, any educational psychology/psychometric assessment prior to school placement is likely to have been arranged via the services which were the precursors of CAMHS (which went by all sorts of names but the records should still be held by the current MH Trust).

When doing a SAR, information about other people than the subject (you) will be redacted. If the dates you were looking for heavily feature mention about your parents, they may been easier to leave out entirely rather than redact parts of.

I made this thread because i wanted to know what it said on the notes i showed. I could have added it to my original thread . But i wasn't sure if I would have got the answer as I think so many people just read the op. Then dont answer the most recent question. But things are spilling over from my original thread. But mainly by me because I keep forgetting it's a separate thread.

Well the answer to the question of why you went to the school, which you did mention in the first sentence of your first post on this thread, is far more likely to be found in the records held by the mental health services, SS or education department/school records than GP records (even if they were complete and legible, which they aren't!). So if you are still interested in the answer to that question I would try those services if I were you. (I have not read your previous thread I am afraid, so you may already have been given this advice, if so, sorry for repeating it).

Op, i hope you dont mind me asking why do you so badly want to know the reasons you went to Sen school? Is it because you had a bad experience in your school or think you would have done better in a regular school.

Information i have are dischage records. But regardless the information is still there but missing for the other children. I had 3 children i hospital and 3 home births to be honest im not overly bothered about the birth information. It just makes ne thibk what else is missing.

I dont thibk it would be CAMH back in the 80s -early 90s but I was thinking something simlar

I dont know how to ask gp to look further into things without sounding rude. She already advised in looking on an NHS site that I mentioned a few posts up. But that doesn't look helpful at all .

Please see my earlier post about what to ask your GP to do. They need to contact the central records people.

You won’t seem rude by asking - it’s part of their job !

The only other thing you can do is go on the website for NHS England and enquire to see if they have any more records for you.

She was wrong. As a receptionist she probably isn’t involved in record management.

if you have been given copies of what the GP holds they should have given you everything they have. It looks like a chunk of information is missing - this will not show up on your NHS app as it’s the GP surgery that inputs it.

That’s why they need to ask the PCSE to look. Only your surgery can do this. You can’t request it yourself.

Have you always been at this surgery or have you moved? If you have always been with this surgery then the surgery have an issue as they have lost your notes as there would have been no reason to send them off to the PCSE. If you’ve moved surgeries then it’s PCSE’s responsibility to try and track down the missing notes.

Yeah i have been advised with a mix of things from gp , to the actual school, to the council, to mental heath type places. I done the go because it seemed easier for me to do . So I think my next step will have to be via education department/council 😔

I understand that as I have changed GP over the years some of my records may not be held with (name of surgery) I have been advised to ask if you could ask PCSE ad this may be where my records are held. I am amazed that you found this record as the NHS usually keeps paper records for finite periods mostly up to 25 years. A huge scanning exercise took place a decade or so ago but the indexing/search functionality was desperately lacking.
In the 1970s and 80s children were sent to special schools for all sorts of reasons that wouldn’t be considered as a good enough reason today. For example, a medical condition such as epilepsy or a mild physical difficulty. There were even places for children who were considered “fragile”. It varied across the country. I hope you get some resolution OP. I agree with PP suggesting local authority records (if they can be located).

I’d actually just give them a ring first. Try speaking to the secretary or someone in admin and if they can’t help then one of the managers. Someone at the surgery will have the job of dealing with registrations and deductions so will regularly deal with the PCSE for record movement. Its unlikely a receptionist will do this unless it’s a really small practice.

They may not need it writing (I don’t at my practice).

Tracking down missing records isn’t something I do on a regular basis but still probably have about 6 a year. I’ve got two on the go at the moment! We have 10,000 patients so bigger surgeries may do more.

Honestly, it’s not an odd request and the PCSE will employ people who just track down missing records. They are a massive organisation who do much more than look after records (for example GP practices get all their blank paper prescriptions from them).

We recently took over a small village surgery (2000 patients) and I got the tedious job of checking all Lloyd George records off against patients that were registered. It was shocking - I must have sent at least 200 sets of records back to the PCSE for patients that were no longer registered. So it’s highly likely that your missing records are sat with a surgery you were once registered with.

Try contacting your local education authorities archive or record office as they should have copies of school records. I’d ring in the first instance to find out who you need to speak to.

If the school was run by a charity you could try them.

I assume the school you went to is now closed down or you could ask them.
