Can anything be urgently done about a pile?

I’ve felt a lump around my anus earlier in the week, and woke up with severe pain today. It was coming from the area so I felt it and it was now grape sized. DH looked, it’s a pile. Blue in colour and external- so can’t be pushed back in.

DH went out and got me Anusol and the pharmacist advised me to take paracetamol and ibuprofen for the pain. Granted, I’ve only had two doses of each and only applied Anusol at 9ish this morning- but the pain is steadily getting worse.

The pain is a solid 8/10. I have an 7 month old and can only feed her laying down as it’s severely uncomfortable to sit up with her. I can’t walk without crying, let alone be on the floor playing with her.

I have heard about piles before, never had one- and just didn’t realise it can be this severe.

Is there a private option to get it drained if this level of pain continues? I can’t believe such a tiny thing is stopping me from doing day to day tasks :(

Have u tried vicks vapour rub? It literally shrivels them up and gives u cool relief i mean do it at ur own risk but it saved the day for me told someone else and they got massive relief from it too. Like a miracle

I’ll be doing that if today’s jaunt isn’t successful!

baby is sleeping and not screaming

i feel awful leaving her but I have been a terrible
parent today and I need to try to do what I can

Appointment with surgeon tomorrow morning- doc has told me to not get my hopes up as they don’t always operate in acute situations. I’m just pleased to be seen and in the system.

The GP who called me said nothing about referrals. Just that it ‘self resolves’

I have found this flowchart online which suggests that I am at the stage 3 or 4 (no idea because no one has seen me) and in the phase of ‘conservative management’ (sit on a bag of peas and take painkillers). It’s been thombosed since Saturday so 14 days from will be next Saturday. After 14 days of
conservative management, I should get a secondary care referral where the treatment is banding or removal.

I will call back next Friday if it isn’t self resolved.
I don’t really know what else I can do.

I am a nurse, OP, not my area of expertise but I do think you should go to A&E. This is unimaginably difficult for you with a young baby. Possibly one of the interventions in the flow chart would be done then and tbere/as a day case. Many A&E depts have urgent care centres attached... you would be triaged by a knowledgeable person. I understand how difficult this is for you with a young baby. Have you got someone who can come with you and c/o your baby with you there to feed as necessary?

overruled by another doctor

i have been told to take to bed and sit on ice until the swelling goes down

i asked how I would do that and look after a 7 month old baby - no answer

was then recommended more painkillers that are not suitable for breastfeeding or co sleeping

no plan of action

very frustrated and angry - and in pain

Have you got another nearby A&E OP? This sounds like such bad advice in your situation.

Please seek a second opinion OP. I was repeatedly dismissed with a “pile” that was actually an abscess. It eventually burst and left a fistula which is not something you want to have.

I wouldn’t leave. It’s outrageous to expect you to carry on like this.

I wouldn’t leave the hospital.

Consultant surgeon here - ask nicely to see the consultant for a review. If it is a thrombosed pile then I too would advise ice, instillagel, laxatives and painkillers (the only painkiller you shouldn't take whilst BF is codeine, there are plenty of alternatives you can be prescribed such as oramorph plus ibuprofen/naproxen/diclofenac). There is no evidence for operating acutely. If however you have a perianal haematoma (different to a pile but looks similar) then drainage may help relieve the pain, although again the evidence is that operating acutely actually drags out recovery for longer. It sounds like you need someone to explain exactly what the diagnosis is and to help you understand why rushing into surgery may not be the best option.

I got none of the fuckers when I was pregnant so god knows why this appeared a month later

i have taken all of the stuff the doctor prescribed for poo problems but the weird thing is- I don’t have any!!!! I have no pain when going to the toilet nor constipation because it is THAT descended and thrombosed.

It’s just chilling outside. The loo is one of the situations where the fucker doesn’t actually hurt

naproxen has definitely taken the edge off but that makes me slightly worried that they are just going to leave me with the fucker if I’m ‘not in pain’ any more

Just seen this post op. I got slight piles at the end of my pregnancy and by the time i had gave birth i had 4big balls on my butt. Doctors at hospital gave me nothing had to get dp to go to the chemist for cream. I couldn’t do nothing, had to roll in and out of bed and i was the same, sitting on the toilet gave me relief. A few days of using the cream did help tho, they deflated and eventually shrivel to nothing

I am staggered at what is happening to you.

I had thrombosed piles on several occasions and each time there was absolutely no fobbing me off and certainly no question of medical advice without even having been examined - appalling bad practice. GP saw me straight away, excised the clot and I got up and walked out pain free.

As I have posted before, on one occasion when there was more than one, I was taken to theatre for an anal stretch - which did the trick.

What has happened to the simple concept of actually examining and treating patients? I am gobsmacked.

My doctor OH has died; but I know he would be utterly shocked at what is happening. The whole idea of treating a patient whom you have not even seen is beyond belief - and allowing that situation to go on for so long.
