Help me to decide whether to go to A&E in the morning

Briefly...have now had period for 20 days straight. Seems to be getting heavier as the days go by...bright red, no clots, not like normal period blood, used 9 pads in a 10 hour shift on Monday.

Am 43. Quick history of recent menstrual issues as follows:

  • Age 36 - periods become thin, pale pink, last three or four days.
  • Age 38-42 - periods disappear entirely.
  • Sept 2019 - periods return, light and pale pink again, last three or four days.
  • Last four months - considerable discomfort and bloating around ovulation in left ovary with masses of egg white mucus being produced over a two week period. Over the last six weeks ovulation appears to fail, no egg white mucus produced, appears to try again, huge bloating, constant need to urinate, once again fails to produce ewm, period comes.

20 days later, it's still going. Over the last five days, tenderness again in left ovary area...uncomfortable to press on lower abdomen. Strong need to urinate every half hour. Dehydrated. Fast heart rate. Nausea. Dizzy on standing. Sleeping 12 hours a day when not working.

Triaged by emergency clinic over the phone as no gp appointments left. Recommends A&E. Would have to get bus, 12 mile journey, really can't be arsed. Fell asleep sitting up at the dining table whilst considering this - that was 11am, woke up at 2pm. Went back to bed for a few hours (day off today and tomorrow.)

What are the chances of this period stopping soon? We are a tier 3 area and whilst I'm not bothered about the virus itself per se, I really don't want to rock up at A&E whimpering that "me periods mekking me reyt gippy sicky like" in the middle of a sodding pandemic.

Can any lurking medics or people with similar experiences advise?

My thanks in advance.

PS: family history of early menopause (40's)

Hopefully you’ll get the MRI done quickly.

titchy · 05/11/2020 16:47

@Cailleach

Two large cysts - one on each ovary. Oddly the bigger one -8cm - is on the right ovary where there is no pain.

Need MRI but highly doubt that will be done today. Just want to go home now.

Has the other burst?
BoreOfWhabylon · 05/11/2020 16:56

@Cailleach so pleased you're getting sorted out.

You have told them about the leg pain, I hope?

Thinking of you OP, and hope you get to go home with good news and a plan of action soon Flowers

Cailleach · 05/11/2020 17:20

Bore, yes I mentioned the leg pain.

Itchy, am obviously not medically qualified but would doubt they would be visible on a scan if they had ruptured.

Hope they are going to discharge me asap...want to get home now.

SistemaAddict · 05/11/2020 17:37

I'm not sure they'll send you home with that amount of bleeding until it has settled. Have they given you anything for the bleeding? Is there a plan apart from an MRI?

Cailleach · 05/11/2020 17:54

I have had tranexamic acid and norethisterone which seems to have largely stopped the bleeding already. One of those was via a drip. I am waiting for the doctor now, should be another half hour or so.

Well that was worth going now wasn't it
And no-one said you were wasting time
I am really glad to see you did go.

Cailleach · 05/11/2020 17:59

Yes it was...you just feel stupid turning up at hospital with a heavy period, but clearly there are some issues after all. Apparently the cysts are not "simple" ones so need further scanning...this will mean more to you than me, Doug, but I am guessing this means that they aren't just fluid filled bubbles, as per usual.

Thanks for your advice last night... always good to hear it from a medic!

OP posts:
DougRossIsTheBoss · 05/11/2020 18:04

Yes that's it. Not just a fluid filled bubble but what they might be will require an MRI and/ or maybe a biopsy to determine.
They might be secreting hormones that made you bleed excessively.

DougRossIsTheBoss · 05/11/2020 18:09

Over 4 or 5cm is considered large and it's advised that over that size they be removed due to risk of twisting.

I've had a Torted ovarian cyst myself and I really think it was the worst pain of my life. Way worse than childbirth. Possibly rivalled by the dry socket after root canal experience.

DougRossIsTheBoss · 05/11/2020 18:11

Titchy isn't wrong either. Sometimes they leak rather than outright burst and that causes pain and bloating etc and may explain both why it was smaller on the left and why it hurt.

Cailleach · 05/11/2020 18:13

Very nice doctor just turned up....my god they look bloody young these days, I'm getting old! - they want me to stay in over night, have ca125 tests then meet the consultant in the morning.

Could be a dermoid or endometriotic cyst, they aren't sure. She mentioned surgery :(

What a ballache :(

I have to hang out with these baby Drs making me feel old all the time.

The day that I told one of them to put his coat on or he'd catch his death of cold was the day I realised I am officially old enough to their mum now. He didn't put it on either.

WhoWants2Know · 05/11/2020 18:21

It will be good to know for sure, though.

I'm fascinated by dermoid cysts, although I might be less curious if it was actually happening to me.

TheMShip · 05/11/2020 18:24

Ca125 is a marker for ovarian cancer. You're very young for that, so it's quite unlikely, but they'll want to rule it out. Though I have to say it's not a perfect marker and the MRI/biopsy will tell the team more. I'm not an oncologist, I analyze ovarian tumour genomes, but have heard enough research clinicians present their work to know the basics.

colouringindoors · 05/11/2020 18:25

So glad you went in OP and are getting good care - you definitely need it. Take care.

Cailleach · 05/11/2020 18:27

Ha ha ha yeah half my team at work call me Mum because I'm always fussing over them! Depressing innit, getting old!

I never Google medical stuff on general principles but having just googled dermoid cyst I realise why...OH MY GOD THE HORROR!!!!

@themship

I had ca125 dove recently and it was slightly raised over 34, is it anything to worry about because I'm still waiting for my scan letter (over one week). No bleeding like poor op, but very tender and sore around period and after. Then it goes.

Goodness op your in the best place! Also best to get in and out of hospital now.

I had a dermoid cyst (although they called it a teracystic tumour). I was fascinated! The consultant said it contained hair and teeth but they are just passed when they don't come across an eyeball! 😆

I think they're amazing! I was insanely lucky that although mine was large (small melon sized) they were able to refer me for it to be done endoscopically because of my personal circumstances.
Cailleach · 05/11/2020 18:56

Oh my god - melon sized?!!! I cannot imagine... Just picturing mine having eyes and teeth now...bit disturbing!

justasking111 · 05/11/2020 19:14

My cousins had hair and teeth, she was 11 when she collapsed in pain. Back then they said it was a twin that had never developed.

randomer · 05/11/2020 19:17

Wish I hadn't seen that

TheMShip · 05/11/2020 19:29

@NullcovoidNovember sorry I don't know enough to answer your question. Fx everything is ok for you.
