Tiredness on Wegovy

I’m two weeks in on 0.25 Wegovy. Whilst I can see a small appetite suppression and had one a couple of days where I hat to force myself to eat, it’s not done much. But the tiredness is weird! General fatigue and after eating a very protein heavy lunch - I eat really well generally - I literally feel like I have to sleep. And often have to have a nap. Is this normal? It’s depleting

I have been on Wegovy for 3 months and have experienced constant fatigue and lethargy…feel like im walking through treacle every day. I have had good appetite suppression right from the start and have stuck at 0.5 mg and lost a stone in weight. However, my regular exercise of gym, swimming and running has gone down the pan as ive just been too tired. I am therefore planning to stop a bit earlier than planned and focus on getting my fitness back up as it feels important to maintain muscle strength and the health benefits of fitness as im in my 50’s rather than losing that last half a stone.

I agree with this completely. Same age group, same concerns - I was lifting really heavy in the gym but Wegovy has really hampered this, which feels counterproductive

It’s really common in the first few weeks, particularly if you’re sensitive to GLP1s. I was wiped out the first couple of months, but it did get better.

I’m 2 years into Mounjaro now and after a recent low end HbA1c blood test, I’ve bought and been wearing a CGM (continuous glucose monitor) this week and eating normally to see what’s happening when I get that wiped out want to go to bed feeling after a meal, and I’ve seen that sometimes after eating I get quite a sharp glucose drop.

I injected Saturday morning and had an Indian curry (the works, high fat) Saturday night, and my CGM very briefly dropped into hypo territory (3.3 mmol/L), which left me absolutely wiped out, blurry/heavy eyed could have done with a nap. for a few hours afterwards.

Other meals such as salmon and green veg (high protein, very low carbs) see glucose dips, lunch such as a cheese and ham toastie (higher carbs) showed a steady rise and fall.

I’m wondering whether something similar could be contributing to that sudden post meal exhaustion for you. GLP1s alter gastric emptying and glucose regulation, high protein or high fat meals can slow gastric emptying even further, meaning the carbs are released more slowly, while insulin is still being released in response to the meal potentially causing the dip in glucose.

Smaller, more balanced meals with protein, fibre and some carbs might be worth trying while your body adjusts.

I do eat really balanced meals, though. Always a lot of proten, some fibre/carbs, and greens. It does feel VERY specific after meals. I literally feel like I need to droop to the floor. I hate it.

I guess then it needs to be a balance of the impact of your current weight on your health (or any other benefits you were looking for from the GLP1) vs the impact of the side effects on your health. It will be a different balance for each of us.

For someone like me who was severely obese 2 years ago, and the impact that had on my health, it was worth continuing to see if I got past it and it did lessen after a couple of months.

I found the fatigue difficult to cope with when I first started on a glp, I did improve and energy levels increased after a few months. I have recently transitioned to a tablet for maintenance which I take every other day. The suppression is perfect for maintaining weight and as I am only having a small dose the side effects are minimal now. Have you considered trying a tablet? It’s not for everyone but I have found it really good so far.

I don't think I can cope for a few months. It's draining me, I'm only 3 weeks in. I don't have a huge amount to lose, I don't think I will continue. It's really like wrestling with a huge wall of tiredness almost all the time.

Yes, I totally understand it's about balancing all the needs. I do not have a huge amount to lose - I was only just viable for the offlicense wegovy at 25 bmi. I think it's up to me to try and lose the weight and not go through the side effects.

I had this as well- it was just about manageable on 0.25 but every time I tried to go up I just wanted to sleep, it was dreadful. I am quite high energy and do lots of exercise normally, so I came off it.

I didn't have loads to lose so it was ok, I still dumped a fair amount of weight on 0.25. But it wasn't worth it to me.

I was very tired the first few weeks, not really related to meals but was heading to bed very early (fairly soon after dinner I guess so maybe related to that after all?)

im now 9 weeks on and it has worn off, and I’ve lost 1.5 stone so far (still only on the second level dose).

Yes, that's probably a good description of me - high energy - and not worth the effort. I'll see how I feel in a week or so. But I don't want to stop exercising! I've worked hard to get fit/strong

I think you are probably right to reconsider if it’s worth feeling so tired when perhaps you don’t have a great deal of weight to lose? I was at 130 kg when I started last May and am now 75kg so the health benefits outweighed the side effects and I felt so much better as my BMI reduced. I really think that the boundaries are being pushed with this medication in that perhaps it’s not suitable for small amounts of weight loss? The impact of these side effects on a person who is feeling relatively healthy must be awful. I know I couldn’t have done this without medication but if I just wanted to lose a small amount of weight I would perhaps look at alternatives options. I hope you feel better soon.

Hi, I'm Zeinab Pharmacist and mum here. Tiredness comes up in my clinics every week, you're not imagining it.
Three usual suspects: you're suddenly running on much less fuel than your body is used to. You're probably drinking less without noticing, and mild dehydration feels exactly like a wall of tiredness (a rehydration sachet can rescue a bad day). And big protein-heavy meals are the slowest to digest on these medicines, so spread the protein out and add some slow carbs like oats or beans.
If it isn't settling, or you feel dizzy or faint rather than just tired, speak to your prescriber before deciding anything. This does usually ease after the first few weeks.
