BMI under 27 - Off Label/less to lose

ritala · 29/03/2026 12:17

I am mid 40’s and just can’t lose the weight like I used to. I’m fed up of losing and gaming the same two pounds. I’m about a stone and a half more than my happy weight (where I would be approx bmi 23).

I have tried to find a thread for people who have been approved ‘off label’ Wegovy or Mounjaro with less to lose but couldn’t find one, so thought I’d start here. I have a BMI higher than 25 but less than 27 with a couple extra health conditions. Hoping to start after Easter. I’d like to stay with a very low dose if possible. Half a pound a week loss would be good for me. I really want to avoided side effects.

Anyone who is in a similar boat and wants to share experiences or ask questions, please join! X

I did 3.75mg again this week and literally feel nothing. It’s frustrating how different it can be week to week, I’ve no doubt our own cycles will impact it but they probably haven’t bothered exploring that very much because it only impacts women 🙄

kljdhs877 · 11/04/2026 10:42

(By nothing I mean SE, still getting suppression, although not as strong as last week)

You’ve not reached the lowest therapeutic dose yet, which is 5mg, which is lkely more to do with it than your cycle. People can get hungrier round their cycle. For some hormonal some psychosomatic, but I’ve not even heard anecdotally it stops the drug working.

maybe it’s more you need to move up?

Thanks for having me! I am starting with Voy, my first dose is due in the post today!! I am nervous but also excited. My blood pressure is rocketing and I need to do something fast to bring it down so I’m hoping that weight loss will help to stablise it. I felt guilty for ages about even considering WLI due to people saying it’s only for diabetics and obese people, but for me it is about getting my weight under control so that it (hopefully) has a knock on effect for the BP. So I’m aiming to do my first jab tomorrow! Eek!

Took my first Mounjaro injection last night. Well actually around 12:30am as I was massively overthinking it. Mainly the actual injection itself but it was so simple and the needle barely even counts as a needle, it’s tiny! I feel fine this morning, will see how I get on throughout the day. Aim is to stick to 1800 calories which is a 500 deficit for me.

I’m due to do my first one tomorrow. I’m very nervous about doing it wrong but hopefully it will be straightforward and I’ll feel better after doing the first one. I still can’t get over how extreme it feels to injecting, but hopefully I’ll get over that soon

Im not calorie counting. Admittedly I’m probably not losing as fast as I could if I did (about 12lbs in 2.5 months) but I’m just eating what I want. The main reason I’m doing this is I don’t want to feel like I’m on a diet, I find calorie counting miserable. I know what I should and shouldn’t be eating, the jabs enable me to naturally make choices better.

I don’t I tend to calorie count either (I absolutely hate it) so I’m hoping I can make good choices without. My mindset going into this is that I need to be able to sustain this way of thinking about food for the long term (ie the rest of my life) and there’s no way I will want to calorie count forever so I’m hoping I can just learn how to make good choices not only on what food to eat, but portion sizes as well. Dunno how that will work in reality but we’ll see!

Sorry what I meant was I did 3.75mg last week and felt awful, really bad SE, and this week I feel absolutely fine. It’s been like that through this, some weeks SE, some weeks nothing. Having no issues with appetite suppression. I doubt I will ever get up to 5mg tbh. I’m only 5lb from target which I’ve done almost entirely on 2.5. Only reason I upped to 3.75 is because I have a holiday next month and want to shift this 5lb a bit quicker 😂

Yeah it’s stopping me from snacking which is my downfall. My main meals are fine. I’ve decided I’m just going to stay on it, not looking to change my habits, I don’t need re-educating, I know what I do right and wrong. If I can maintain my weight on 1.25mg a week, curb my IBS symptoms and make my appetite manageable it’ll absolutely be worth the £75 a month (buy a 2.5mg pen every 8 weeks).

Yep, depending on how I react to it I may well end up doing the same. How does it work with buying a pen and using it over 8 weeks though when they expire after 30 days?

kljdhs877 · 11/04/2026 11:22

@KittyQuestions34 basically going against the guidance! Lots of people say the 30 day thing is overly cautious, they don’t go from safe to unsafe overnight. I keep it in the fridge even though it doesn’t really need to be. Voy have started allowing 6 weekly orders on the understanding pens are being used for over longer periods.

FeelingSadToday1 · 11/04/2026 11:41

Can I rejoin? I started 18 months ago and lost 3 stone in 8 months on 2.5mg. I then microdosed at 1.25mg for a bit, then maintained on my own at 57kg.

I have recently had a long course of high dose steroids and have put on weight and have started bingeing a but again. So yesterday I opened my emergency pen! I was 60.75kg this morning and I feel sluggish and fat round the middle (also an apple shape). I only took 1.75mg last night and already feel less hungry, calmer and more in control. Hopefully I will shift this 4kg and be back on the wagon as I won’t be able to order again :(

A lot of people are buying higher dose pens and using them longer than 30 days to save money. My current pen is going to last me about three months, which is a risk I’m prepared to take. I’ll obviously keep it refrigerated the whole time and be scrupulous with hygiene!

Issue comes In when reordering, after this long most make yoh go back to 2.5.

I think as long as you say it’s within 2 weeks of your last dose, you don’t have to go back to 2.5. I sometimes would leave it 3 or 4 weeks between jabs at high doses.

On here if you quote a post it means you’re responding to thay post. And the poster was speaking to 3 months.

Therulerofmyhouse · 11/04/2026 15:24

I’ve just taken my first dose today of the 2.5.

My starting weight is 72kg and hoping to get down to 60kg. My BMi is about 28 although when applying they all claimed my BMI was 27 and initially got rejected by Bolt pharmacy but accepted by MedExpress without needing an interview.

I’ve had a hellish perimenopause and weight has steadily crept up despite being a regular gym goer and walker, I tend to eat pretty healthily but do succumb to the wine witch. But no matter how much I cut back or up exercise I can’t seem to lose weight so am hoping this will help. My metabolism has definitely changed since being in Peri.

andrinaballerina · 11/04/2026 15:58

Interesting to hear about not calorie counting, I was hoping I could take this approach, but also want to be fully equipped once the weight is off and I try to maintain.
Regardless I’ve taken the plunge and have just had confirmation that my order with Voy has been approved. I’ve gone for Wegovy and plan to start after a work trip next week. Hearing of others with similar BMIs and weight to lose has given me that push to go for it!

Yes true. I’m not aiming to be on it for long as I don’t have much to lose, and I have three pens stockpiled which should last me a good few months if necessary. But yes I will have to start back at 2.5 if I ever want to order more in the future.

CatAsstrophe · 11/04/2026 17:13

I'm new to the thread, thanks for starting it 😊

I agree that having a low'ish BMI (in comparison to many) and using WLI can result in negative judgment. I've only told DH and won't be sharing this info with anyone else.

I've been on Wegovy for 4 weeks, purchased from Voy. My BMI at the start was 27.1, start weight 11 stone 9lbs. I've lost 8lbs in 4 weeks. I decided to start using WLI as my weight has started to creep up - I already lost over 5 stone years ago (not with WLI but with diet/exercise)

Ideally I'd like to lose 1 stone 9 lbs in total. Or perhaps a couple of pounds more to get slightly under 10 stone. But, as I'm 60, I'm worried that losing too much weight will be ageing, so I'll wait to see how I look.

The side effects have been very unpleasant. I've chosen not to increase the dose and to remain on the starting dose of 0.25 for the next 4 weeks. Hopefully, the side effects will ease off soon.

ritala · 11/04/2026 17:53

I’ve had my first jab today!!!! I was so, so nervous and it took me about two hours to pluck up the courage to do it, but I did it. It was just so unnatural and as mentioned before on this thread ‘extreme’. I was like, “Am I really doing this? Do I really need this? Ritala, WTF are you doing?”

It goes against everything natural to inject myself with a drug. It took me another two hours to actually calm down. Just awful anxiety. I feel ok now though and have had a cereal bar for some sugar (my body just felt like it needed it?!) and I have my flask of water to hand.

it got me thinking - I was on the contraceptive pill for decades, literally over 20 years and until the last couple of years, I never really questioned it. Yet that was me pumping myself with hormones, literally taking a pill every day to affect my hormones? It’s crazy I did that without even realy thinking about it and I’m getting hugely worked up about this now.

I wonder if that’s how women felt in the 60’s/70’s when the pill became popular? Maybe future generations won’t think twice about jabbing for weight loss?!

I am so glad Shot 1 is DONE!

Oh I remember that the first time is always nerve wracking, the next few times are similar but gets less each time. Now two years later I don’t even rhink about it. 😂

andrinaballerina · 11/04/2026 18:58

ritala I’m going through the same mental gymnastics Grin thinking I’m mad to be starting this, but I’ve decided I need to give it a try!

ritala · 11/04/2026 19:03

@andrinaballerina- we are all in this together! It’s totally moral to feel anxious - that is what makes us human and it’s your brain trying to keep you safe. That’s what I was telling myself!

Feel the collective positive energy of all of us here. You have got this!
