BMI under 27 - Off Label/less to lose

ritala · 29/03/2026 12:17

I am mid 40’s and just can’t lose the weight like I used to. I’m fed up of losing and gaming the same two pounds. I’m about a stone and a half more than my happy weight (where I would be approx bmi 23).

I have tried to find a thread for people who have been approved ‘off label’ Wegovy or Mounjaro with less to lose but couldn’t find one, so thought I’d start here. I have a BMI higher than 25 but less than 27 with a couple extra health conditions. Hoping to start after Easter. I’d like to stay with a very low dose if possible. Half a pound a week loss would be good for me. I really want to avoided side effects.

Anyone who is in a similar boat and wants to share experiences or ask questions, please join! X

Re taking different doses from pens, I'm thinking about potentially doubling my dose this week and taking 0.5 from a 0.25 pen ( as my new one has already arrived due to switching provider and I'm stepping up with that so no reason not to start early really). Would I just do two jabs, does anyone know? Or can you turn up to 0.5 as one dose on a 0.25 pen, can it go past the 0.25?

Thanks!

IDontKnowWhatYouMean · 20/05/2026 19:40

I'm just about to start my second month on Wegovy. I've had two 4-day weekends of partying in the first month, but have managed to lose about 9 lbs on the 0.25 dose.

I'm with Voy and have gone with their recommendation of the 0.5 pen for month 2, but I only want to increase a little bit from the .25 dose I started with. I'm hoping if I can make it last 6 weeks instead of 4, I'll be at target and won't need to make a third payment (I'm not committed to a contract with them).

Is anyone here already successfully click-counting? To make a 4 week pen last for 6 weeks, I'd need to count to about 25 clicks each week, is that right? I've looked at the calculator and asked ChatGPT! Thanks for any advice about how to avoid clicking pitfalls 😀

That's exactly what I'm doing. 25 clicks so 6 doses at 0.33mg so a little higher than previous dose.

You can't turn it further so 2 jabs.

Are you just coming onto week 3? You might find it quite a jump if so, it builds up in your system over 5-6 weeks.

maybethisway · 21/05/2026 07:36

Morning all! I’ve just weighed myself and I’ve lost 2.4kgs in 6 days 😳 I think I need to work on eating a bit. Having said that, I’ve seen others have big losses in week 1 and then it stabilises at a more reasonable rate. Looking forward to the nice weather arriving 🙌

Thanks! No it will be week 7. I would do a midway dose but don't think I can really do that with what I've got left.

I read the whole thread last night and would love to join your group. I am 55 (5’2 and 67kg) I am hoping to get to 60kg.
I had my first jab on Sunday night and so far no side effects 🤞. I lost weight previously during covid by calorie counting but have managed to pile it all back on. Snacks and alcohol are my downfall. I ordered my pen from MedExpress, had to send a photo of me and ID and they have notified my doctor as it’s now on my health record.
I am definitely snacking less this week but am trying to eat little and often.
This is my second attempt at the jabs, I tried Ozempic over three years ago but it made me feel so tired and nauseous, I couldn’t carry on. This time I am jabbing my thigh instead of stomach and hoping to keep the side effects at bay.

Welcome! I started last Friday and have also had no side effects. Alcohol was y big downfall too - I drank lots of calories and it stopped me being consistent with diet and exercise. I had 2 drinks last weekend and felt awful so I won't be doing that again in a hurry. The good news is that I am totally uninterested in alcohol (previously I had quite a lot of 'alcohol noise' to use a WLI term). Good luck 😊

lechatnoir · 21/05/2026 17:22

Just jumping on this thread in case anyone is in the early days and feeling disheartened - Four weeks on the 25mg Did absolutely nothing for me but I increased to 0.4 last week and then 0.5 yesterday and in that time I’ve lost 4lbs and seen a noticeable drop in the food noise. I think for some people, it just might take a little bit longer to kickstart or you might need to move up to that higher dose, but I guess it’s just trust the process And hopefully you’ll get there too.

Fourkgtogo · 21/05/2026 19:06

I’m just wondering what weight you all want to lose in total and what were your loses in the first month?

badkitty · 21/05/2026 20:04

@lechatnoirthank you for that - I’m in my first week and a bit worried that I’m not feeling that much appetite suppression at the moment - a bit less food noise I think but I doubt I will lose anything this week. No side effects other than fatigue though which is a plus!

I'm 5'5 and started at 73kg officially but probay closer to 71kg if measured naked first thing in morning.

I'm 67.5kg now after 4 weeks, and aiming for about 63kg, in the middle of the healthy bmi range. I don't want to be very slim, a small 12 will be great.

It does build up over the weeks. Its good you don't have bad SE, you can always increase.

I don't have much suppression but it's enough to follow a diet so that's OK.

Proteinpudding · 21/05/2026 22:08

I'm only in my first week after dosing Sunday, and I don't feel the major appetite suppression like I've seen some people write about - I'm not nauseous, or put off food, or forgetting to eat, or feeling like I have to force food down. But, and this feels like absolute magic, I'm eating the meals that I've always planned to eat - and actually sticking to them. I've always been interested in nutrition, but I would find myself sacking off the chicken salad and fruit I'd taken to work and buying meal deals instead with extra chocolate to go with it, or eating far too much pasta or rice with my evening meals. This week i've been satisfied with my healthy meals, and small portions, it's been amazing. I know my weight loss will be water weight this week but I've felt fine on 1200-1400 calories, I've done my usual gym sessions and dance classes and surprisingly not felt tired or lacking in energy for those (which was a big fear for me, and I've used as a justification for over eating in the past)

I'm astonished I could feel like this from a 0.25 wegovy stater dose. I don't know how long it'll continue but I'm feeling very lucky right now, and I hope other people experience similar!

I'm 5ft 2 and started at 64.5kg (though my consult was in the afternoon, I'd usually weigh in the morning when I'm a little lighter!) I'd like to get to under 60kg I think though I'm not sure exactly what weight would suit me. I want to keep a decent amount of muscle as I've strength trained for a long time.

JustBlossom · 21/05/2026 22:29

Little update from me…I’ve just gone up to 0.5 of Wegovy this evening…hoping the side effects aren’t too bad as I was managing a steady loss on 0.25 (5 lbs / 2.27kg as of last week - I weigh on Friday mornings) but food noise had been creeping in. I don’t feel
like I can “see” the loss as yet but appreciate I have less to lose so have to be patient.

Ideally I’d like to lose another 10lbs to get to target weight - I have a holiday in mid July so hoping that’s achievable if I stay consistent with exercise and steps too.

What’s really surprised me if how much better I am sleeping - I would regularly wake up throughout the night with a noisy brain worrying about all sorts of minor things that definitely weren’t important at 3am! But that’s totally stopped.

Fourkgtogo · 21/05/2026 19:06

I’m just wondering what weight you all want to lose in total and what were your loses in the first month?

I want to lose 7kg overall and I’ve lost 3.5kg in the first month from microdosing MJ (started 0.5mg first week, 1.0mg next 2 weeks, 1.5mg 4th week).

I’m now on 1.5mg (wk 5) and hope to reach target weight anywhere between weeks 8-10.

Then I’ll titrate back down for a month or so and then hopefully stop.

slartibartfastina · 21/05/2026 23:55

@JustBlossom I have also found WLI have reduced my middle of the night fretting and anxiety generally. I just feel calmer generally.

I am 5’2 and started at 69 kg/ 10 stone 12. In my first month I lost 7lbs/3 kg. That included some water weight as since then I have lost another 4lbs/1.8 kg. My goal is to get to 62 kg/9 stone 8 ish. More rewardingly, I’ve lost almost 2” off my waist and 2” off my hips and am fitting easily into all my old clothes again. I have been on a bit of a plateau since the beginning of May but I’m trying not to get discouraged.

aquitodavia · Yesterday 07:15

I started at about 11 stone, I'm 5ft 6. I want to get down to 9 and a half, I lost about 10 pounds in the first month though slowed down over the last couple of weeks and only lost another couple of pounds, so I'm about 10 stone 2 now. I'm going to step up my dose now and hope I can get down to my goal in the next 6 weeks ish. Then I may stay on a maintenance dose, not sure yet.

That’s interesting. I did a full dose last week (I’m due to jab again today) but I’m thinking of doing a smaller dose. I’ve lost 2.4kgs this week and I’ve not been hungry at all. I have eaten obviously but it’s hard at dinner time. I haven’t told my OH what I’m doing and we’re going away with the kids this week; just in the uk to our holiday home so we’ll be eating similar things but we’ll be together all the time so it’ll be obvious if I’m barely eating. Decisions decisions. Maybe I’ll do half today and see how I go.

jazzflute · Yesterday 09:05

I’ve just finished my first month on Wegovy and I decided to continue on 0.25mg for the second month. I’m feeling a bit depressed this morning as doing my fifth jab and haven’t lost any weight the past two weeks - down 2.5kg overall since starting. I am not tracking food and did have a big event last weekend but didn’t drink or eat nearly as much as I usually would. I also don’t think about food and feel the appetite suppression is still there. Have also been quite busy and stressed the past two weeks so will try to track food this week and see how it goes! Would love to know if this happened to anyone else who continued on 0.25 the whole time and if the weight loss still kicked back in

lechatnoir · Yesterday 10:10

I'm 5ft 6 and started at 12stone 2 now 11st 6 after 5 weeks -slow start as I didn't dropping a lb for the first 3 weeks. My target is 9st 7 but it's been a long time since I've been that weight and I'll have zero boobs by then & might look old and haggard😩so initial target is 10stone and then see how I look/feel.

andrinaballerina · Yesterday 10:50

@jazzfluteI think we’re on a similar schedule! I’m due my fifth jab now and have lost a total of 2kg in the past four weeks. I think slow and steady is going to work best for me; I’m not tracking calories, but finding it easier to stick to my planned healthy meals without the urge to snack. I’ve decided to stay on my 0.25 dose for another month, and will perhaps consider going up next month. For the moment I’m enjoying not having to give it much thought, while steadily losing weight, and having no side effects.

happymumday · Yesterday 10:59

Happy Friday everyone, I did jab 7 of MJ last night. I managed 2.5mg for the first 3 weeks but have been on reduced doses ever since, trying to find a level I can tolerate (anxiety, shaking, jitters, horrendous fatigue, headaches, vertigo & nausea). I can just about function on 1.25 but sadly very little weight loss on this. I have tried so hard to push through, but I have thrown in the towel and asked Voy to switch me to Wegovy. Really hoping it suits me better and I can get settled on an effective dose. I have lost 4.9kg in the 7 weeks so far but most of that was in the first 2 weeks. I feel really disappointed that most people appear to get on with it much better than me, but will start Wegovy with hope🙏

NinthBestOption · Today 09:41

I've been musing about my weight problems and the effects of the jabs beyond making you less hungry so you can maintain a calorie deficit.

I'm over 5 weeks in now, really strict on diet, 1100-1200 cals a day. I eat chicken, fish, eggs, veg, skyr, seeds, berries, pulses. In the last 5 weeks I've had 2 glasses of wine, 3 chocolate biscuits, a little pizza and a slice of cake. No crisps, chocolate, sweets, juice, junk food, etc.

Its been pretty easy to stick to even though I get hungry and don't have that full feeling. Yet I stopped losing weight after week 2, since then I'm losing and regaining the same 1lb. 10 years ago the weight would have fallen off me.

I've no thyroid problems, I am on hrt, I'm as active as I can be, I sleep well and minimise stress. Yet there's obviously a problem.

The jabs also work on insulin sensitivity and reducing inflammation. I wonder if this could be my issue. I'll step up my dose next jab and see.

RubyTraybake · Today 09:46

I’ve been feeling nauseous this week and have had two incidents of being sick in the night. When I’m feeling rough I really question what I’m doing to myself here in order to lose weight! I thinking of stopping, 7 pounds from target but 1.5 stones from starting weight and using the rest of the pens I have to keep to this weight if I end up putting on a few pounds on holidays etc. I am really happy with the 1.5 loss as I was struggling to lose anything pre MJ, it’s just been a painful journey!
I wish it worked as well for me as it’s seeming too for other people but I don’t think it’s meant to be!
