BMI under 27 - Off Label/less to lose

ritala · 29/03/2026 12:17

I am mid 40’s and just can’t lose the weight like I used to. I’m fed up of losing and gaming the same two pounds. I’m about a stone and a half more than my happy weight (where I would be approx bmi 23).

I have tried to find a thread for people who have been approved ‘off label’ Wegovy or Mounjaro with less to lose but couldn’t find one, so thought I’d start here. I have a BMI higher than 25 but less than 27 with a couple extra health conditions. Hoping to start after Easter. I’d like to stay with a very low dose if possible. Half a pound a week loss would be good for me. I really want to avoided side effects.

Anyone who is in a similar boat and wants to share experiences or ask questions, please join! X

I'm using Superdrug as you can collect. It was a bit fiddly but I took the pictures using a round mirror at an angle propped at waist height. Also take one of feet showing weight on scales.

Thank you!

Beachforever · 11/05/2026 14:15

@aquitodavia that’s amazing loss, well done! You must be so pleased.

It’s day 7 of week 3 for me and I’ve not lost any this week after losing 2kg the first 2 weeks. It’s disappointing but I’ve had a virus all week, spent most of it in bed and haven’t exercised at all so kind of unsurprising.

I’m moving up to 1.5mg MJ from tomorrow so hopefully will get back on track.

NinthBestOption · 11/05/2026 14:29

Bit of a loss for me this morning so hope I'm off the plateau.

My pen has a bubble in the last dose. They tell me it's harmless, I'm not too happy! I have been doing the flow test with each new needle, turns out you need to oy do it with each new pen. I think that caused it.

New needles - you can buy them online, mine say 4mm 32g insulin needles I think. Or probably your local chemist can help if you don't have the needle label to check.

andrinaballerina · 11/05/2026 14:41

I am on day four of my third jab and really feeling it this week, in contrast to last week when I hardly felt any difference. I need to decide today whether I want to up my dose for my next pen, or stick to 0.25mg for another four weeks. Last week I was convinced I would go up, but after feeling pretty nauseous all day today, with good suppressions all weekend, I’m not so sure now.
Anyone else swung between all and nothing from one week to the next?

RubyTraybake · 11/05/2026 19:06

1000 calories today plus a large glass of wine. Struggling with exercise, I’m in my early 40s but feel 60 with all my aches and pains so don’t feel like I have any energy left! I blame my young children. I tried on a load of clothes at the weekend and it made me feel rubbish so I clearly need to keep going with the weight loss.

@andrinaballerina the dosage in your system goes up each week even if you stay on for same dosage. I would go up to 5mg and then click count if you want to have a lower dose as it works out much cheaper

aquitodavia · 11/05/2026 19:33

Does anyone have a good guide to click counting that they can link to, please? I am thinking about going up and click counting too.

NinthBestOption · 11/05/2026 19:44

@aquitodavia this one was recommended for Wegovy https://wegovyclicks.replit.app/

I too plan to stay at 0.25ml from a 0.5ml pen until at least week 6. Pens are good for 6 weeks from first dose which means you can get 7 doses out of them without ignoring the guidance, I think my 0.5ml one is £140 so £20 a week not bad at all. Once I'm not tied to Voy should be able to buy cheaper.

Thanks all I will wait until I’m back from holiday. I’m having a wobble to be honest as I suffer anxiety and i keep seeing awful stuff about panic attacks. Has anyone noticed increased anxiety?

Sensible to wait I think. I wasn't aware anxiety was a side effect, not experienced any changes mentally. I guess I worried in general about side effects but nothing much happened.

lechatnoir · 12/05/2026 09:20

Interested to Understand more about the clock counting As I’ve got a 0.5 mg pen arriving today and feeling apprehensive about going up straight to 0.5 But I keep my pen out of the fridge, so not sure I can stretch it over five doses (Which if I’ve understood it correctly, I could count 30 clicks five times to give me 0.4 mg) Anyone know if this is an option Or should I just bite the bullet and go with the 0.5? So far, I’ve had no side-effects or obvious suppression.

OneBusyFinch · 12/05/2026 13:02

I went with Voy and my application has just been approved. I will be using mounjaro.

i have some more questions if i may ( appreciate the order and website should answer them but so good to ask people who are actually using them)

  • What’s it like injecting? Is it straightforward (have never injected anyone or anything until now)
  • when in the day is it best to inject? Start of day? Before bed?

all advice welcome!

gracefulprocrastinator · 12/05/2026 13:27

@OneBusyFinchit is very straightforward. The instructions are really clear and easy to follow. I tend to jab on a Sunday evening- it’s worked well for me so if there is any socialising on the weekend requiring me to eat, I can do that without making things too obvious as that’s when the suppression is lowest. Honestly I’m so happy with the progress so far. Good luck!

Beachforever · 12/05/2026 13:32

@OneBusyFinch

Injecting is absolutely fine and easy, an I say that as someone who is terrified of needles. I think the fact that the needle is so small and it’s not going into a vein or anything, just a pinch of belly fat helps me think of it differently to other injections/needles.

I take it middle of the day on a Tuesday. I haven’t had any side effects but I do it then just in case as I work from home and so would prefer to deal with any SE’s when I’m alone in the house! Also it gives me maximum suppression over the weekend when I’m more likely to indulge in booze or heavy meals.

aquitodavia · 12/05/2026 13:42

I also do Sunday, late morning to afternoon.

ritala · 12/05/2026 17:44

Hi all, sorry for radio silence. All is good with me, I am click counting from a 0.25 Wegovy and have been doing a 3/4 dose (28 clicks) so 0.18. It’s really helped with my insomnia and I am sleeping much better. However, I am no way feeling the suppression and just feel pretty normal. Maybe not quite as interested in food but nothing like week 1 where I could barely eat. I know the way I am now is the more sensible way, but I must admit - the strong suppression was good from a ‘calories in’ point of view.

I am going to carry on click counting for now. I do my jab on a Sunday evening for the same reason as @gracefulprocrastinator. I like my weekends to be a little more ‘treaty’ and also don’t want people to know I’m on the pens, if I can help it.

ComeTheFcukOnBridget · 12/05/2026 22:40

Does anyone know if the pens lock or is that just a myth to stop click counting? The Voy AI assistance is telling me it does lock after 4 uses, but I'm not sure I believe it...

OneBusyFinch · 13/05/2026 00:19

My mounjaro is being delivered tomorrow

SandwichesAndGingerBeer · 13/05/2026 05:18

@ComeTheFcukOnBridget oh no really?

@ritala you’re click counting aren’t you? Has your pen locked?

RubyTraybake · 13/05/2026 07:32

@ComeTheFcukOnBridget my pens lock after 4 doses but you can use a needle to get the last dose out, it’s really easy. I know some people force the pen but it didn’t work for me.

After no loss last week I’m 3pounds down this week 🥳

RubyTraybake · 13/05/2026 07:34

@ComeTheFcukOnBridget sorry it’s after 4 full doses so 240 clicks. If you are click counting it doesn’t lock until you’ve done all 240 clicks and then you use a needle to get the ‘5th’ dose out.

NinthBestOption · 13/05/2026 10:31

Week 4 day 1, I managed to get the bubble out of my pen and took the last dose, it's probably a bit short but can't be that bad. New pen arriving Friday.

Weight loss had plateaued, got moving again recently but I'm having a heavy day today. I think the trend has settled at about 1lb a week which I'm happy with so won't increase my dose unless I really need to..

Side effects are minimal, my sleeps fine now, rarely any nausea, 'throughput' has speeded up a bit.

Wanted effects are subtle, I'm still generally hungry and can always finish my plate, I still think about food but I'm generally less interested when it's in front of me so no real desire to overeat or raid the kids snacks.

Sticking to 1200 cals a day max. I'm 5'5 and currently 68kg, that should be a good deficit so I'd say only losing 1lb a week does suggest there's something 'wrong' with my system. 10 years ago I'd be dropping 3 or 4lb a week on that.

slartibartfastina · 13/05/2026 16:23

I have been on WLI for 9 weeks now and have lost 10 lbs (10 stone 12 to 10 stone 2), and 1.5” off my waist and 2” off my hips. I increased a bit from the starter dose last week because I was feeling a lot hungrier and finding it harder to keep to 1300-1400 kcal per day. Generally I have found it super easy and the bad SE minimal. I am sleeping much better and have less anxiety generally. It is true that a plate of cheese and crackers isn’t as delicious as it was before but I’ll take a small reduction in pleasure on that front for everything else! My weight loss has slowed down though, it was about 1.5 lbs per week at the start and is now just under 1 and I had a 10 day plateau where nothing moved, probably my old friend, water retention.

SandwichesAndGingerBeer · 13/05/2026 17:10

Fallen off the wagon completely today 😫. Awful nights sleep, got my period and in the office next to too many treats that my hormones and tiredness are making it too hard to resist!

OnTheTop · 14/05/2026 07:38

How are you feeling today @SandwichesAndGingerBeer? Hope you managed to sleep better last night.

I’m feeling pretty good this and looking like another decent weight loss, which is so motivating.

I’m out for dinner on Friday evening which I’m a bit apprehensive about. Just the idea of eating much later than I’m used to and more food than I’ve been having. I don’t want to be uncomfortable afterwards.

maybethisway · 14/05/2026 09:42

Hi Guys! I have not started yet but hoping to receive my first pen tomorrow. I wondered how everyone has felt about exercising? I have done a far bit of running in the past and keen to get back to it but not sure how I will feel. I appreciate the SEs are very individual but wonder how everyone has felt about exercising?
