3.75mg MJ

Hi, I am just about to go up to 3.75mg from my 5mg pen,had quite bad nausea from 2.5mg but that subsided and last couple of weeks have been fine. I didnt want to jump to 5mg just before xmas hence the transition.

I just wondered what most people do if they use the 3.75. Do you do for a few weeks then go to 5mg, or do you just use the pen for longer than 30 days but ensure its in the fridge and you are super carefully with hygiene?

You don't need to use the pen for longer than 30 days. 3.75mg from a 5mg pen is 45 clicks. There are 240 clicks available in the pen, so doing 45 means you can do 5 doses from the pen (45 * 5 = 225). 5 doses fits within 30 days - days 1, 8, 15, 22 and 29.

Yes, I’m using a 5mg pen and taking a 3mg dose each week as I didn’t want to increase to 3.75mg or 5mg just yet.

I keep the pen in the fridge in a plastic bag and I’m happy to risk it going over the 30 day limit, but you have to decide what level of risk you’re happy with.

I’m still ordering regularly from my supplier so as not to arouse suspicion as the ‘use by’ date is into 2027. However, I did talk to my GP about it (they provide the prescription) and he seemed fine with my plan as he gave me another 6 months worth of 5mg pens in my latest prescription.

There will be over 5mg left (golden dose) so I was thinking from that point of view

Yes this was my thinking also, I dont really want to be on it forever but want that option so need to just monitor when I am getting low/close to reordering deadline.

Does your GP know you are using the pen longer than 30 days then? They didnt say anything?

I am comfortable with the risk I think and will check it carefully for cloudiness etc.

You've been clear of nausea from 2.5mg for a couple of weeks. Can I ask what is driving you to increase your dose, and do you really need to go up? How long have you been taking mounjaro?

@HappyWineDay TBH the issue is that I ordered 5mg as was following instructions from the pharmacy when the emailed me to reorder, until I had read lots on here I didnt realise people didnt do the suggested increases.

I am still losing weight and not that hungry on 2.5mg until day 6 and 7.

Good question from @HappyWineDay, if you are still losing at a decent rate (1-2lb a week while still obese, a little less once overweight or normal) there is no need to increase at all, back 18 months ago providers encouraged patients to increase when they didn’t need to. There are lots of advantages to staying at the lowest dose that works for you.

cross posted - I would stick to 2.5mg then, take 5-6 weeks (or whatever risks you are willing to take with no evidence of safety beyond 30 days) from the pen and discard the rest.

you are already winning by getting extra weeks so don’t see the discarded liquid as a waste in your risk assessment, the priority is your health

My BMI is under 27 now and still losing 2lb+ a week although that is bound to plateau soon. I'm a little annoyed that the pharmacy's still suggest this without asking questions, I put how much weight I had lost on 2nd consultation also.

I can stay on 2.5 but obviously its such a waste of the medication if I cannot use it for 8 weeks once open.

Another option is to order another 2.5mg pen, and keep the 5mg pen for later when you do want to move up. The expiry date for an unopened pen should be many months from now.

Thanks, yes they are 2027 expiry so plenty of time to use.
