Query over extracting doses with a needle/microdosing.

Hi everyone,

I have had an up and down journey with symptoms so have had to drop down doses. However, I have a 7.5mg pen nearly finished and another in the fridge of 7.5mg which is my scheduled pen. I also have a 2.5mg pen that was offered by my supplier with £25 off as the prices went up. I aim to use this last to titrate down on 1mg per week as I can no longer afford the pens and have roughly a stone - stone and a half - left to lose, and hope I can do so with what I have remaining. I was fine for several weeks and then bam omg...🫣 a lot of pain and discomfort.

I did 5mg via clicking for three weeks from the 7.5mg pen and then last week dropped to 2.5mg again via clicks due to awful side effects. The side effects have been the same but the last few days I have felt OK so am going to continue. I am about to do my fifth week on this pen and sticking to 2.5mg. I understand that the pens are only supposed to be 'good' for four weeks; it is a choice I have made and am OK with it. What I would like to know is when the pen no longer clicks how much do I extract for 2.5mg from the 7.5mg pen via a syringe as if I am doing the 'extra' dose. I know there are many people micro dosing so what do you do?

In summary:
7.5mg pen
Week one 5mg
Week two 5mg
Week three 5mg
Week four 2.5mg

How much do I extract 2.5mg with a syringe and how much do you think I have left in the pen.
How many weeks with a full 7.5mg pen would I get at 2.5mg per week and how many weeks with a 2.5mg pen on 1mg per week?

A 7.5 mg pen is 12 regular doses of 2.5mg.
You’ve currently got 12.5mg left in your pen (regular).
That means you’ve got five 2.5mg regular doses left, each of 20 clicks.
After that you need a syringe.
Then you should be able to get three 2.5mg doses by withdrawing 0.2ml each time.
All of that is my opinion, at your own risk.

Thanks! The pen will no longer work for clicks so I have to resort to the syringe from now on. There is a lot of liquid left.

The pen will still have plenty of clicks left as you have only used partial doses. It should not have locked after only using for the doses above

Each pen has 240 clicks available, or 2.4ml excluding the leftover medication. You've used 140 so far from your 7.5mg pen (40 x 3, plus 20) so you have 100 clicks or 1ml remaining, excluding any leftover medication which would probably be an additional 0.6 to 0.9ml.

So from your current pen, you could do another 5 doses of 2.5mg at 20 clicks before the pen locks. If you want to use a syringe, you could do those 5 doses of 0.2ml, and then another 3 or 4 at 0.2ml using the leftover medication.

Of course that would take you way past the 30 days usage for the pen, and I wouldn't take my workings out as correct, you would need to check the maths and the reasoning for yourself.

For your full 7.5mg pen, each normal dose of 7.5mg would be 3 doses at 2.5mg (20 clicks or 0.2ml). So you could get 12 doses out via click counting at 20 clicks, and then use the remaining medication to extract another 3 or 4 doses at 0.2ml.

Regarding your 2.5mg pen, a 1mg dose would be 24 clicks or 0.24ml. You could do 10 doses of 24 clicks before the pen locks, and then you would have possibly 3 doses of 0.24ml using the remaining liquid. (0.24 x 3 is 0.72ml, from the remaining medication which is usually between 0.6 and 0.9ml)

Again, I wouldn't take my workings out as correct, you would need to check the maths and the reasoning for yourself.
