Hi,

I am not a rule breaker at all (but am a worrier!) so think this may be why I'm panicking! I've been taking MJ for 6 months and I'm currently titrating downwards.

I've had four doses from my 5mg pen and have a 2.5mg waiting for me to go onto. I was given seven needles with my last pen (no idea why!) instead of the usual four. That, combined with the fact that I'm concerned at being back on 2.5mg over Christmas and piling the weight back on, lead me to take the fifth dose for the first time. I primed then twisted hard to 1 and injected. I've then googled after the fact and worried myself as they say the concentration can be higher (I don't really see how!) Surely I've not had significantly over 5mg? If I twisted to 1 then that is just a normal dose isn't it? I really don't want to make myself ill! I will stick to the rules next time!

The extra needles are just spare as rarely they can be faulty, plus they are extremely cheap to provide so a couple spare isn't an issue for the prescribers.

I'm fairly rule compliant but I am also fairly relaxed about using the remaining liquid in the pen as a fifth dose. The concentration cannot alter, so if you have done a full dose to the 1 position then you'll have had a full dose from the pen. So 5mg. There isn't enough liquid left for much of a higher dose. Even people who extract it with a syringe get 0.7ml ish where a full dose is 0.6ml.

@SilenceInside Thanks for the reassurance. My logical brain says that it was fine and I haven't overdosed but my compliant brain was saying rules are there to be followed and I was worried!

Your post wording rang a bell. Are you basing it off that simple pharmacy article where it says?

"Weight loss injections have been carefully measured to ensure the correct amount of medication within each dose. However, in the extra buffer liquid, this might not be the case – due to the volume not being as measured, the concentration of Mounjaro that is contained within the buffer might be higher or lower than the required amount. This can lead to under or over-dosage, each of which carry their own risks."
It is poorly written imo, they don't mean the concentration of Mounjaro in the buffer liquid is different (it couldn't be), they are saying if you take more/less volume the concentration (dose would be a better word) of Mounjaro you get will be higher/lower. So as long as you take one dose of 0.6ml either using pen hard twist or a syringe you are good.👍🏻

@WeAllHaveWings Yes! That's the article I read and it made no sense to me that it could be more concentrated. It's definitely poorly written as you say.

How did you even get the liquid out? I tried twisting it and it would twist out like normal?

I use a syringe for the bonus dose and it's usually 7/8mg rather than the 6 of a normal dose.

mL not mg, sorry it’s an important distinction to make for anyone reading this and wanting to measure their own doses from the extra liquid in the pen.

.6ml of the solution gives you whatever mg the pen dose is (.6ml from a 5 mg pen is 5mg, .6ml from a 10mg pen is 10mg, etc)

I wonder if I am the only person never to have used the 'bonus' dose. I'd be really worried it was more than one full dose, and equally worried about using a syringe, from the hygeine point of view. Clearly I am losing out financially, but have peace of mind.

@Adelstrop entirely your prerogative but if you use the pen, all you can get is 0.6ml same as usual and if you use a syringe, you can measure how much you are drawing/injecting.

On the hygiene front, how would a syringe be any different to a normal pen needle? They are sterile in the same way as a pen needle (as long as you are not an idiot and buy the ‘measuring devices’ from Amazon!). I understand some people worry about it blunting (never noticed a problem myself) but if you swab the bung in exactly the same way as usual, how could it be unhygienic in any way shape or form? I have complete peace of mind about using it.
