Mounjaro extra dose- dosage advice please.

I read a thread in here which I now can't find, which told you how much a Mounjaro dose equalled in an insulin needle.

I bought some needles to extract the extra dose from the pen and managed to get '80' out. But what is that equal to?

Each full dose is equal to 0.6ml (60) on a syringe. So 80 (0.8ml) would be a bit more than the full dose.

nfg2000 · 29/10/2024 18:56

Thank you! It looked a similar dose but suddenly had a panic that I'd injected too much and started over thinking!

What dose pen is it - 2.5mg, 5mg, etc? It would still be a slightly higher dose than the one you're currently on but if you're looking to step up the dose to a higher one anyway, then it's an okay bridge to the next dose up.

How much is the bonus dose in a 7.5mg pen?

If you're using the hard twist on the pen, then you get the same 0.6ml dose for any bonus dose, but if you draw it out using the syringe, it's something you'll have to find out by checking how much was drawn out.

It's 2.5mg, so titrating up to 5 next week anyway. I remember seeing a table on another thread- how do you know what they equal?

Dose (mg) = (volume (ml)/standard volume(ml)) x standard pen dose (mg)

Your dose = (0.80/0.60) x 2.5 = 3.3 mg

Assuming you use a syringe and get 0.80 ml out too...

Dose (mg) = (volume (ml)/standard volume(ml)) x standard pen dose (mg)

Your dose = (0.80/0.60) x 7.5 = 10.0 mg

@EliLilly Thank you - my maths is shocking. I think I understand. Where do you get the syringes?

What's the hard twist on the pen please? I'm nervous to try and get different syringes but would like to be able to use the extra!

Thank you @EliLilly! Smile

It's basically twisting the pen hard to get a 5th dose from it. It can be difficult for many people so you can use your teeth to twist or pliers or brute force but once it loosens, it's easy to twist to 1 and inject as usual.

The pens officially have 4 doses and the rest is said to be for priming but after you finish the 4th dose, there is still medicine left in the pen and most don't want to waste it. So as a 5th dose/"bonus dose/"golden dose" (you may see ot being called any of these terms), the pen needs to be twisted hard to get it to move again as it sort of locks after the 4th dose.

How soon after last dose can you take the bonus dose?

Conscious it 'expires' after 30 days. But does it really 'expire' or is it a marketing ploy to get you to buy another one a week sooner than really needed?

Thanks

You can take 5 doses within the 30 days on days 0,7,14,21,28.

It is not a "marketing ploy", you can ignore anyone who says that. Unless all the WLI and insulin and lots of other medications in similar form are all in cahoots. 🤔

It is a liquid medication with preservatives and very specific storage instructions. Eli Lilly instruct that the pen should be discarded after 30 days from first use.

Does it mean the medication turns toxic on day 31? Obviously not, but the impact significantly after the 30 days no-one actually knows. The storage and handling of each pen in a domestic environment will be different and each persons response to any changes to the medication after 30+ days will be different so you cannot make decisions on whether to use the pen after 30 days based on, frankly ridiculous, suggestions it is a marketing ploy, you need to do it based on your own research and attitude to risk.

Hi started mounjaro 6 months ago lost 3 stone and was on 15 mg dose. Had lapse and need to start on 2.5 mg and have a full 15mg pen. How much do I draw up in syringe to equate to 2.5 mg

SilenceInside · 21/06/2025 12:46

@aortic60for 2.5mg from a 15mg pen it’s 10 clicks on the pen itself, or 0.1ml extracted in a syringe.

2.5mg is 15mg divided by 6, so the dose you want is 0.6ml divided by 6 which gives the 0.1ml. Please check my maths and reasoning.

aortic60 · 21/06/2025 12:48

Thanks very much

aortic60 · 21/06/2025 12:51

Hi so how much on an insulin shringe please , I’m terrible with maths

SilenceInside · 21/06/2025 13:10

@aortic60well it’s 0.1ml on whatever syringe you have. If you have a 1ml insulin syringe that’s marked up in units up to 100, then it would be 10 units.

How do you extract the extra dose from the pen using an insulin needle? I’ve bought some needles but not sure how to do it and would save some money doing that.

IReallyNeedThisToWork · 08/07/2025 21:31

@newstar1 push the needle into the red bung at the end of the pen. Turn the pen so the syringe is underneath and slooooowly draw out the liquid. It can feel like you need 3 hands but it’s ok, it works.

I release the syringe every so often to let the air sort itself out and just pull, release, pull, release until the liquid is all in the syringe. This can take a minute or two. Don’t rush!

Once you’ve got it all, check how much you have (0.6ml or 60 units on a 1ml syringe is a normal dose) and push out any you don’t want. Tap to get rid of air (not desperate as it is a subcutaneous injection) and inject.

Hi everyone. I have a 7.5mg Munjaro syringe but want to withdraw 5 mg strength. Can anyone advise on amount of fluid I would need to withdraw please. Thank you in advance
