I realise you have detailed all of her symptoms and issues, etc, but did you focus on the extra care you then need to give her because of those? Sometimes it can be the way it's worded as they seem to have set criteria they have to follow. So rather than talking about her sickness, for example, literally spelling out what you need to do for her as a result. I know it should be obvious, but I think the clearer you make it for them, the easier it will be to assess.
