Should I end our engagement over his refusal to manage his health?

Engaged for 6 months, been together over 2 years. Don't know if I'm getting the ick or being picky or what it is. I've known how he was about his health pretty much from around 6 months into dating him, I hoped he would take a different stance, but it's driving me up the pole now.

This will be the second time around for both of us. Both divorced. We are in our early fifties. I am in fairly good shape, very sporty, decent diet with the occasional night of a few drinks and a takeaway, but mainly lots of healthy food and have cut right back on sugar earlier this year. Well woman check last year showed everything to be good, the only medication I am on is HRT, otherwise I would be homicidal. Don't quote me on that 😂

Fiance has high cholesterol and is borderline diabetic. It keeps pushing over into type 2 numbers but then goes back down, The GP is checking it once a year, which I personally don't feel is enough. He has been on a statin for nearly 18 months and it got his number right now from 6.5 to somewhere in the just under 4 region, which is brilliant. Howwwwwwever, a few things are happening.. still happening.... and he is so stubborn about taking advice and taking care of himself that I am on the brink of ending the engagement.

His sugar and fat consumption are ridiculous. They have been for many years since before we met, so it's nothing to do with me or my behaviours. Despite knowing he has high cholesterol and is on the statin, it's almost as if taking the statin leaves him free to just consume as much saturated fat still as he wants. He fries chips in beef dripping, which he hasn't changed in over 2 years, he refuses to drink semi-skimmed milk, eats entire packets of salami for lunch, washed down with a pint of full fat milk, pours all the fat from meat onto his plate, even if he has grilled bacon, he pours the fat onto his bread. This is why his cholesterol was 6.5. I've tried to explain to him that the statin is getting the number down, but ideally he should do it himself. My Uncle had the same issue and we helped him change his diet and he didn't need a statin in the end.

He refuses to change and makes vomiting sounds and pulls a face when I put skimmed milk into my tea.... which leads me onto..... his sugar consumption. When I met him, he was putting 4 heaped spoons of sugar into his tea, which he makes super strong in quite a small mug, no teapot in sight... and of course puts full fat milk into it. He drinks over 10 to 15 cups a day like this, sometimes coffee, which he puts lots of sugar into as well. If a packet of biscuits are in his house, he will eat them all. Same thing for a cake meant to feed a family.. think the Asda boxed cakes. He has a compulsion to finish them.

I know a fair bit about diabetes as I lost a good friend to complications of the disease, and I hate to go on about it, but I suppose I'm trying to make him realise that yes, he could lose a leg, his eyesight, his kidneys could pack up. Right now his kidney function is OK, but if he keeps on keeping on, it won't be. His blood sugar is 47, which I believe is right at the top end of the borderline range. He has cut back the sugar in his tea, but is still consuming half a jar of jam per day on bread, smeared with thick butter. Yesterday he finished a large jar of jam that he only bought 2 days ago.

His attitude is that there is a pill for it and if he does get diabetes, he will be OK if he just takes it. I've tried to explain what can happen on that pill, and that your pancreas can eventually give up and you end up on insulin for the rest of your life.... or worse. He says he will do something about it if it gets worse, but this is already the 11th hour in my mind, or for anyone with an ounce of common sense about their health.

The latest development is he has jaundice in his eyes, which I noticed a few days ago. He noticed it too. He also has blurred vision. Of course it can mean many things. But the statin could be damaging his liver, it could be a bile issue, something else. But jaundice should never be ignored. I've urged him to see a GP. He hasn't made an appointment. He also has extreme fatigue that seems to be getting worse and worse. I almost drove him to A&E yesterday as I was so worried.

I'm sure I don't even need to mention the ED that happens due to diabetes, but he doesn't even seem bothered about getting the number down for that. I thought that might sway him, a reason to change, but it hasn't.

Today I have decided to back off. I woke up and felt so angry that he doesn't care enough about our shared future to take care of himself better. I'm starting to feel like his mother and not his partner and that's not what a relationship is about.

What do you like about him at all . ? No physical stuff, gross eating habits and sounds very uneducated. Does he have money 🤑. Any money he does have will be paying his Care Home fees .

Could you ring 111 on his behalf and then get them to talk to him, as the @theDudesmummy suggested he needs medical treatment if he has jaundice.
Maybe this could be the rude awakening he needs.

Ok so two issues. The immediate one is the jaundice. I don't have enough medical knowledge to know if it's serious but it seems so based on doctor response above. How can you bribe/threaten/cajole/deceive him into getting it addressed? Use his kids? Drive him there under pretence? Threaten to move out (temporarily if he doesn't go)?

the second issue is his attitude to his health. People can change, but they have to want to themselves. If you become Nagging Nurse it's going to kill your relationship even if his health doesn't get worse because it changes the dynamic and puts you in critical parent mode. So it's worth thinking about options for way forward. It might be a serious discussion, maybe with a therapist. It may be that you have to let go and accept him the way he is (with all his amazing other qualities), it may be he has to hit some kind of life/health rock bottom. But the current approach, you cajoling, isn't working? Or is it bit by bit? Has he had therapy? Why does he value himself so little? Or is he a fatalist 'when your time is up, it's up'? Is he emotional eating? I would be curious to explore his beliefs first before challenging them.

Jaundice can be a painless symptom of pancreatic cancer. Not always but it can be and it was, in my family’s experience of the disease.

But clearly some of his behaviour is pretty bad given that blood sugar result eg eating that much jam a day.

I think refusing to seek medical treatment when his eyes are jaundiced just sounds ludicrous, what a plonker.

I dunno that I would be thinking about marriage (unless perhaps he is well insured). But perhaps you can still see each other without getting married?

The gluttony food and sugar addiction I see as an issue for him to sort and I have some sympathy as I am obese myself - however I am always trying and would
never mock and almost goad my partner with it.

However the non teeth washing is not just grim for him but is almost like a fuck you to you - anyone would manage their personal hygiene as it impacts others. It’s a different dynamic to the impulsive and compulsive drive of his food addiction - it’s just gross and anti social - would he go as far as not wipe his arse?

Theres a resistance dynamic going on here over and above the food - what was his relationship like growing up with his parents - it’s like hes taking some revenge out on you.

Anyway ask yourself why you have allowed yourself to be so disrespected by this man for so long and why with such clear unaligned values you are pursuing this relationship. If your standards, boundaries and self esteem are so low you need to get some help fixing yourself and not be preoccupied fixing him.

I also don’t understand that you say ‘his weight is OK for his frame’ but he carries lots of visceral fat. BMI doesnt take account of frame size - are you also minimising his weight issue?

He has... had... elevated cholesterol caused by too much fat. He was put on a statin because of this, but continues to consume full fat everything. On top of high blood sugar, it's a recipe for stroke, heart attack etc. etc...

I agree. Jaundice is a symptom that needs urgent medical attention. Happened to someone I know and turned out to be cancer.

yes yanbu and this would give me the ick too, how’s your sex life and how’s his weight and fitness?

on another note, it sounds like something very serious could currently be indicated. Don’t want to scare you but could be a pancreas thing, perhaps the big C (not medical but personal experience). He needs to get that jaundice checked out.

It's not your business if OP wants to drink semi skimmed or goats blood. If she's 'fussy' you're being 'judgy,' and missing the point.

Ok no need to go off the deep end. My view was it’s a mixed picture - he’s bang out of order in some ways and clearly doesn’t take much care with his health. But I am sceptical that swapping to skimmed milk will make much difference to his cholesterol (compared to eating all the bacon fat from the pan, say). He should obviously not make stupid noises when OP drinks whatever kind of milk she fancies.

It was very obviously just one part of a much bigger picture of his very unhealthy lifestyle, not to mention his rudeness and immaturity.

I wouldn't be able to respect someone who lives their life this way. Let alone be attracted to them. If you've made it clear to him how you feel and he still does nothing about it, you only have one option. Do you think if (IF) he made drastic changes now, you'd stay with him?

I would say yes, put it off, then try to pull away. Be wary of getting suckered back in if you find out he is seriously ill. He sounds like he is on the edge of a crisis.

Some of the things you are concerned about, I could overlook, others I'd think were disgusting and/or negligent. But it's obvious that you nagging him to take better care of himself is not having the desired effect. And he doesn't see himself as being in any kind of health jeopardy. Since this seems to be a big issue with you, I can't see it changing unless he has a major health scare that causes him to re-evaluate his habits. But even then, he might believe that medical interventions rather than lifestyle changes will do the trick. So unless you want to continue nagging him and worrying about him, and perhaps caring for him in ill health, then I'd probably make good on your threat and walk away. Who knows? It might be the scare he needs to make a change. But if he does "change" you need to wait to see how embedded and permanent it's going to be before caving in again. Good luck.

You might want to talk to him about how he feels about being on advanced life support and his medical wishes if he's incapacitated and given he's jaundiced and got blurred vision, you probably should do that today.

Sadly OP painless jaundice could mean pancreatic cancer, in which case his pre-diabetes and raised cholesterol will be irrelevant.

So he's just been to visit his sister and I said get her to have a look. She agreed that it looks like jaundice. Now he's booking a GP appointment, finally.

You sound fundamentally different. You are clearly intelligent, informed and diligent and he is not.

In general I think if you even have to ask a question like “should we break off our engagement” you already know the answer.

The jaundice thing could be as serious as liver failure. I hope he’s ok.

It depends really how much you love him and whether being with him outweighs the idea of caring for him then losing him early. The transactional person in me thinks you could invest this time in finding someone with a more similar outlook who may not require care and may be around longer, but that is a pretty harsh way of looking at it.

He hasn't taken care of his teeth sadly, so yes they are in a bad way, but love is blind etc. He mocked me once for cleaning my teeth twice a day and flossing.

I was was wondering about his personal hygiene but his dental hygiene in particular. Honestly Op, the poor teeth cleaning would have me heading off into the distance let alone his diet.

someone has asked " Is he rich" ( as in being tje only sensible reason to marry someone destined to die soon if youre lucky). But you could help things along, of course, you could insure his life against sudden death, which is quite cheap, say for a few million, then indulge his stupidity, feed him crisps and chocolate and stodge, get him seriously overweight and unfit, then suggest a night of passion with maybe a few girl friends to help out, by the morning youd be a rich widow.

If it's been a while since his last hba1c rest I bet he'll be diabetic by now.
Honestly how is he going to manage his diabetes if he has such a cavalier attitude to his health
Hope when he goes to the doc she will lay it on the line kindly but firmly.
As for you, if it was me I'd honestly give the ring back. It's one thing if you're type 1, or working hard on controlling your diabetes, but he thinks a tablet will cure him, it won't, it may mitigate the symptoms but it can't perform miracles
You could still stay friends and maybe reconsider if he has some sort of road to Damascus experience and gets things under control. Until then I'd seriously think hard about what you'd be signing up for
