When did everyone receive their MATB1 form? I know it can only be issued from 20 weeks, but I just wondered if your midwife issued it or whether you can get it from your GP? I had my 20 week scan yesterday but I never saw a midwife afterwards to ask about this and my next midwife appointment isn't until when I'm 28 weeks 🙄

Yeh I got mine from my midwife and she forgot to give it to me at one appointment so I got it the next time.

I haven't got a telephone number for my midwife @dementedpixie and the numbers I have just don't seem like the right ones to be calling about this, they're for triage, birth centre, birth suite, antenatal clinic numbers 🤔

Your employer doesn't need the MatB1 until closer to the birth. You do need to have informed them of the pregnancy and what date you want leave to start by the 25th week of pregnancy.

Within 21 days of your SMP start date (or as soon as possible if the baby’s born early) give your employer either:

  • a letter from your doctor or midwife
  • your MATB1 certificate - doctors and midwives will give you this no more than 20 weeks before the due date

You can email for it or just wait until your 28 week appt. They don't need it so soon.

I won't get SMP from my employer so I need the MATB1 to send with my MA form @dementedpixie I want to be able to send it in the earliest date I can, to prevent any delays.

Leave a message for your midwife and ask her to complete it for you and either post it to you or leave it at clinic reception for you to collect.

If I can't get hold of you midwife I call the GP and they leave a message for her or her team and someone picks it up. Can you try your GP?

Try calling your health centre/clinic/GP number as they may have contact details for the midwife team

You can take your notes to the antennal clinic or your GP surgery and they will fill one out. You might need to call ahead.

Your employer will probably want the certificate by 25 weeks. Mine wanted it when I put in my maternity leave notice.

They might say they want it at 25 weeks but they don't actually need it at that point

I could have written this! I phoned my midwife today to ask and they said they would give it at my 28 week appointment so
i had to explain my employer wants it by 25 weeks I need to formally request my leave by then and can’t do it without the MatB1. It seems that employers do want it by 25 weeks so they seem awkward by not issuing it before then.

All they need at 25 weeks is your notification you are pregnant and when you want leave to start. Legally they don't need the MatB1 until within 21 days of your start date.

You dont need proof of pregnancy for leave, it's for maternity pay

I got mine at my 24 week midwife appt, 1st pregnancy low risk not sure why others don't have a 24 week appt I'm afraid

I got mine at my 28wk appointment, I think that’s fairly common. My work just said they needed it before I go off on leave but other than that they were happy to wait til I had it.

I have my Matb1 form, but only part of it is filled I. Do I need to fill anything in on it before handing it to my employer? Also, do you have to get it back from your employer after because it has info for the birth date etc on it? Is the MatB1 the only form you need? Sorry for all the questions!!

Hi @EmilyH91 your matb1 form should be double sided, your midwife fills one side and you complete the other.

If you are applying for SMP I think the only thing you need to give your employer is your matb1 and the date you intend on taking maternity leave. If your applying for MA, you need to send the following documents with the form - payslips, matb1 and smp.

You can claim maternity allowance if you don't qualify for SMP from your employer or are self employed
