8 weeks scan issues :(

Good afternoon.
I had private scan today, I should be bang on 8 weeks today. I have 27/28 days regular cycles and ovulation confirmed by opks on 12th or April, last period 30th of March x
I was told there is 2 sacs and they can see two embryos. Twin A is measuring 3.5mm with visible heartbeat, obviously it’s about 2 weeks behind measurements wise 😩. Twin B is measuring only 2.4mm and there is no heartbeat. I was told to re scan in a couple of weeks.
Anyone has been in similar situation? What are the chances the either one or both will survive? I attached the photo if that’s of any help.

I was 7 weeks when we saw 1 hb and 1 without. 2 weeks later there was 2 hb’s

Putting aside the twin aspect if you are sure on your dates then I'd say sadly the twin without the heartbeat doesn't sound viable sorry OP as at 8 weeks a HB should 100% be visible.

What was the heart rate of the other twin?
Measuring 2 weeks behind isn't good either but is there any chance you could be out with your dates? Or is it possible that you could have ovulated twice a week or so apart? (Very very very rare)

@ivfgottostaypositive thank you for your reply x to be fair that was my assessment of the smaller twin! I haven’t got the heart rate of the bigger twin unfortunately, it was clearly visible though x scan was done internally if that makes any difference x it’s my first pregnancy and I have no idea what’s what!
Is it often than pregnancy is multiple but one of twins doesn’t make it?

Can't comment on the twins issue but it's not unheard of for dates and measurements to be out in early pregnancy.
I had a scan when I should have been 7+5 but baby only measured 6+1. She caught up by about a week by the dating scan and I'm now over 28 weeks, so it can happen.

I want to add that one of my twins were measuring small (around 1-2 weeks).
I found out at my 13 week scan that the smaller twin had stopped growing around 10 weeks.
I hope this isn’t the case for you and everything goes fine 🥰

It can really go either way at this point. Also half a millileter is going to make a huuge difference so it's quite normal for measurements to be out this early.

Do you have any idea what the heartbeat was ? The speed can give an idea of whether it is viable.

Did you have a positive pregnancy test 14 days after your positive opk ? All the opk tells you is that you had a spike in hormones, but you could still have ovulated later.

@Malysh thank you for your reply x the flicker of a heartbeat looked quite fast which I found so surprising x I got a positive test at 12DPO x

@Katie1109 OPK’s don’t confirm ovulation, the confirm the LH surge before you usually release and egg.

In other words, your body can gear up to release an egg but fail to do so (you would still get a positive OPK) and then ovulate properly a week or so later.

It’s an awful situation to be in when you’re in doubt.
I’m hoping for the best outcome for you OP X

@Eskdale2004 my LH surge is always a day before my ovulation, confirmed by ovulation pains on the day and previously temping x also I kept using opks until CD18 just in case, but I’m pretty sure I ovulated 12th of April x

I miscarried twins back in Jan. miscarriage clinic told me that twin pregnancies tend to be less robust but it's unusual to lose both twins. Especially after a heartbeat (mine never developed at all - just empty sacs).

I'd have hope for the larger twin and just keep my fingers crossed for little twin.

You're still very early days so measurements are unlikely to be exact.

I was just wondering now after reading some stuff on internet should babies measure 8 weeks if my LMP is 8 weeks? Or should they in actual fact measure 6 weeks because that’s the time since conception? x

They measure them from the date of your last period rather than conception and that's the universal way of doing it so the measurements they'll be comparing against are the 8 week ones x

They use LMP as an early estimate up until your dating scan but thats based on an average 28 day cycle with ovulation on day 14.
They then go by the actual measurements at the dating scan to give you an official EDD and that should be more accurate.

My LMP date and EDD from dating scan are 5 days different which is right because I'm in a LDR and only had a short time frame where I could have conceived (early scans measured me 11 days behind LMP which I knew was wrong).

Don't forget, if working it out from conception/ovulation, you still have to add 2 weeks on at the beginning as the date is taken from when the egg is released.

Ahhh I’m so confused 😩 I just don’t know what to think x I’m very happy one has a heartbeat and will be over the moon if the one survives, but I’m preparing myself to think that the other one won’t. Sacs also look much smaller. Any thoughts on scan photos? Is it normal or am I right in thinking the smaller one hasn’t got much of a chance? Please be honest, I’m very pragmatic and can take it.

None of us can give you the answers you want - we could all be very wrong. And it's very early days with some questions about dating.

For the smaller twin, the sac size looks slightly irregular to me, which could be a bad sign. Or it could be a weird angle. There's debate about whether an irregular sac means anything tbh - and all x3 of my mmcs had perfect shaped sacs but still mc'd.

Do you know if they're identical or fraternal? This could explain size difference.

@Katie1109 Hi, I just wanted to come on here and give you a little virtual hug. I am afriad I don’t have any advice but I can offer.some support in the waiting game. I went for a private scan that showed I was measuring a lot early than I thought and I have to go back in two weeks to see if the baby is developing. It’s not exactly the same as what you are going through but I understand the waiting and the stress. Xx

@MayBug2020 thank you very much x and so sorry you have to go through it x honestly I read a lot about early scans showing twin pregnancies and then one of twins does not develop and it really does not stress me out, it’s how the nature swings and I cannot do anything about it x I just want at least one of them to develop properly, which is more likely to be the bigger twin with heart beat x how far along you thought you were and what they measured you as if you don’t mind me asking? x

@Katie1109 well it's a bit of a story so apologies for the essay :) I have been on the pop pill for 12 years with no periods and stopped at the end a few months back to start trying. I had my first bleed, which I believe they call the withdrawal bleed at the end of March. So I only have those dates to go by as I don't know my cycles at all. If I went by LMP I should have been about 7 weeks but was only showing 5. I only got a positive test just over a week ago which showed 2-3 weeks but booked a private scan as I thought I was further along. Oh and I should mention I did two tests which would have been 2 and 5 days after my period was due but both were negative (again only went by the app saying my period was due). So I am hoping it is an error in dates but won't know for another 2 weeks which is a killer. I know what you mean and what will be will be as nature has it's own reasons for how things turn out but I can't stop thinking about it x
