11 weeks, passing clots, will A&E do anything?

I am bleeding again and have passed 2 clots, one the size of a 1p piece, one maybe half that size.

It's Sunday and there are no maternity services open. If I went to A&E would they scan me? I went to A&E with bleeding on a weekend in my last pregnancy and they eventually did scan me but this was in another city so I'm not sure whether that was unusual.

I can't wait to know - I have to travel for a funeral next week and just want to know whether the baby is ok or not.

Feel absolutely shite. :(

Oh Hun, try not to stress too much. I would go to a+e and see what they say... Are you in a lot of pain? The clots sound quite small too. I do think it is pants, that in this country we have maternity services only Mon-Fri in office hours! If you have no joy or are asked to wait to long, you can try some private places...or even private hospitals. Hope all is okay, xxx

I have been cramping since the night before last - enough that I suspected I had a stomach bug. Now I'm back to thinking the worst. Will private places be open today then? Will have a google.

Sorry this is happening but having been there myself its doubtful to be honest. Your likely to be kept hanging about for ages, sent up to the gyne ward eventually. Have an internal to be told they can't tell you anything it's just a waiting game and most probably sent home till tomorrow and called back for a scan then.

Best thing to do honestly is rest as much as you can today. Keep an eye on the bleeding and any other losses . If it gets bad (more than a pad an hr) go to a&e. Take paracetamol if you need to, if pain gets too much again go to a&e.

Get in touch with your gp or midwife first thing in the morning. Good luck

Go to a&e or at the worst a drop in centre. They should do blood tests (for hcg levels), and certainly a&e should get you sorted with a scan (may have to wait til Monday for that).

I had a mmc last November and went to a&e twice, and they were great. They also have me the phone number for the gynae ward so I could call them if I had further problems, before i ended up being admitted for medical management for mmc.

I really hope it turns out much better for you. Keeping fingers x

I had a really, really heavy bleed at just after 11 weeks on a Saturday. Went to a&e. They sent me to EPU. Tried to find heartbeat but too early/too flabby a belly so they couldn't find one. So I got scanned instead. Might just have been luck there was a sonographer on shift at the time but worth a go.

My son is now a happy and healthy 8 month old by the way. Fingers crossed all is well for you too.

I can't find anything online about weekend private clinics in my area. I think I will have to to to A&E. If nothing else they might be able to refer me for a scan tomorrow. Still bleeding, got dull, period-type pains. This is after what felt like stabbing pains. I just don't think this week could be any more wretched. Thanks for your advice.

Been in this situation 4 times.
A and may well refer you straight to epu or get an appointment for you to go to epu tomorrow. Epu should either give you an ultrasound, internal scan, internal examination or blood test. Don't be do bed off! Or a and e may do blood tests for your hormone levels.

Thanks, Mummy. I'm hoping they will be able to take action fairly quickly. It sounds awful but I haven't got time for this to be happening. I know my family will all be wondering why I'm not drinking after the funeral and I don't want to say I'm pregnant when it's all gone wrong and I don't know it.

I think it depends what sort of 'action' do you want..?

Usually A&E role/decision making is whether you need to be redirected to EPU if the service is available (normally a Monday-Friday working hours - some places have limited weekend scanning but will probably be gatekeepered by gynaecology on-call for 'emergencies') or whether you need to be referred to gynaecology on-call for consideration of admission to hospital.

I have not heard of any A&Es that do these types of scan themselves (even trauma A&E scans are being increasingly deferred to on-call Radiologists) or gynae on-call doctors who routinely would either - especially in earlier stages - they'd want the people who do it regularly to be making that call.

Also in some hospitals even the hormone level blood test may be limited to only EPU being able to request because the interpretation is complicated and it needs sequential follow-up.
Emergency management is theatre for a procedure not a scan.

Urgent management is admission for close monitoring and scan at next available opportunity.

(But also worth bearing in mind scans give you a 'snapshot' of what has happened so far and sometimes if done early give more uncertainty because they need to be repeated after a certain interval.)

Reasons for admission tend to be - if concerned an ectopic (no previous scan confirming location, clinical history and examination features), if the bleeding is very heavy and haemodynamically unstable (e.g. fast heart rate, dropping blood pressure, fall in blood count), cervical shock (problems with the clots blocking the cervix - links to haemodynamic instability), pain not manageable at home or signs of infection (e.g. temperature, foul smell) etc.

Then the management would be supportive/symptomatic in the interim (e.g. removal of obstructing products on examination/theatre for a procedure, IV fluids, blood transfusion, stronger analgesia, close observation and staying nil by mouth in the interim in case emergency procedure needed) until next available scanning slot. Which sometimes isn't necessarily the first next one - as some units will prefer to queue-bust some of the walk-in/outpatients first while the inpatients continued to be stabilized and are reviewed on a morning Ward Round.

I was hoping at the very least I might get a referral for EPU tomorrow. I have to fly on Tuesday for this funeral and I am concerned that if I have lost the pregnancy this will complicate things with my travel plans. When I attended A&E with problems early in my last pregnancy (suspected ectopic due to previous gynaecological problems) I was referred on the day for an internal scan. They were able to detect that the pregnancy was in the right place (although it was so early they couldn't discern if it was progressing or not) and I don't know whether it is possible with an internal scan to detect whether the pregnancy is progressing (heartbeat or not) to be honest. Anyway I am at A&E now, just waiting to be called.

A scan should show if there is a heartbeat or not at that moment if 11/40 & they may well be able to see on a transabdominal rather than intyernal scan.

But there could still be uncertainties e.g. if bleeding continues anyway (need for repeat scan to see if heartbeat still present) or if everything looks smaller/earlier on the scan then it may be too small to say that there should be a heartbeat seen for sure (so need for repeat scan +/- hormone blood tests).

They have referred me to EPU and told me they will be in contact tomorrow morning with a view to hopefully scanning me tomorrow. Now just waiting and hoping for the best - don't feel very positive though. :(

It's all over. Started to lose a lot of blood this morning. Scan showed still some retained products (such a lovely term) so just waiting for doctor to discuss my scan results with me. I just want it over. In a lot of pain now, like contractions, hoping it will stop before I fly out for the funeral. Feel physically wretched but otherwise numb. :(

Meerkat, I'm so sorry for your loss. I've had a mc myself and it's a hideous experience - give yourself plenty of time to grieve.
