to declare or not to declare

I'd love to know when filling in a form that asks 'Has your child had any counselling - please give dates and details', whether or not you do or don't.
I didn't (form for dd to go on organised trip abroad - she had been self harming and had been hospitalised four years before) and it came back to bite me as doctor's letter had mentioned it.
After to-ing and fro-ing of letters from group organisers and said professional who included me in it (thank god) and also recommended that she be allowed on trip, they have turned her down 2WEEKS before she is due to go.
I am raging about the way this has been handled and how this will follow her seemingly whether we declare it or not.
Sorry to be so long - i don't see how this will help her maintain the ok state she now appears to be in.

I think that you have to declare it if it is true, presumably the schools insurance for trips like this will rely on these questions being answered truthfully.
Seems unreasonable to prevent her from going if the professions involved from the mental health side think she should go. Who is stopping her? Is it the school? In which case would it be worth asking if they would treat a child with a past physical health problem in the same way?
If it is insurance could you pay an increased premium?
