Sertraline no longer working?

Started taking Sertraline 50mg a day in July last year. Changed my life, amazing. Only downside was the very weird and vivid dreams. Which I could put up with as I was feeling good during the day.
As my problems were thought to be possible Pmdd, gp suggested changing to only taking 50mg per day during luteal phase only. As this has been proven successful for pmdd. I was happy to try.
However, it did not work out well for me and after 4 months I went back to taking every day. Have been taking everyday for almost 4 months now, but I am still not feeling better like I was when I was taking everyday at the start.
I don't know if this makes sense or if there's a reason.
I am still having the wild vivid dreams. But I feel really down and not just around my period.
I am going to have another chat with gp and I'm sure she will have another suggestion but just wondered if anyone had experienced similar? And if you changed to something else or changed dose and it's improved? Thanks.

I had sertraline a few years ago but a ridiculously high dose and I couldn't walk with the vertigo it gave me. If you were working well on it, have they not tried increasing the dose or are you on a high level already?
I had an embolism in Jan and ended up having panic attacks afterwards (it stemmed from being ignored when having the embolism and told it was anxiety, ironically) and have been put on beta blockers which are helping with the racing heart. Do you get physical symptoms like that? When you say "down" it sounds more like depression than anxiety, so maybe your root cause is changing and new meds needed?

50mg is quite a low dose OP so it would be worth asking your GP about increasing it to 100mg, especially as you felt so good on it before.
My partner is on 100mg and is much happier and less grumpy on that dose.
