I am hoping some of you might be able to advise.
During covid (2021) the gp rung me to discuss a test result. At the end of the conversation I mentioned I had been feeling anxious with all the covid restrictions etc and she gave me 50mg of sertraline. There has never been any follow up since and they gave me a repeat each month for the last two years. I taped off myself a little while ago and am no longer on the meds.
The medical form I need to complete for an assessment specifically asks if I’ve been diagnosed with any mental health issues (I haven’t ever been diagnosed or followed up). Do I leave it blank? write something on the form like covid anxiety? I don’t know.
It also requires the gp to fill in whether I’ve been diagnosed with any mental health issues and any meds. I have full access to my medical records and cannot see anything on there about anxiety at all. On the date I was prescribed the sertraline there was no notes about it, just that it was prescribed. Could there be more notes on my file I cannot see? If so, how do I access?
I was being prescribed sertraline longer than I was taking it for - do I tell the gp that?
