How do I know why type of therapy I need

I’m looking at going private for depression and anxiety therapy/treatment

I’ve been struggling since childhood and been on various antidepressants for 17 years. They all seem to work for a bit and then slowly be less affective. I’ve had cbt in the past and normal talking therapy neither were very helpful but they were through the nhs so not many sessions.

I’ve been diagnosed with persistent depressive disorder and anxiety. I also have PMDD and SAD.

the depression diagnosis is described as mild depression and how I feel the majority of the time isn’t mildly depressed. I wish I wasn’t here any more tbh, the only thing stopping me is the trauma it would cause for my children and husband.

I feel like there’s something more going on but I just don’t know what it is. I’m exhausted, I’m so tired of fighting to keep going.

So the point of this is I’ve been looking at therapy but I don’t know which I should go for but also I feel like I need a general assessment to get a diagnosis of what else there could be or to confirm the current diagnosis is correct. I don’t know where to start. I looked online but it seems the assessments are all for specific things and I can’t seem to find an all round one.

it all feels very overwhelming so any advice would be appreciated.
