Pip review

Hi there.

I've had the form for pip review. I'm a bit worried about it .

It's for my mental health which is appalling. On the form it mentions any changes to my condition: I now have multiple diagnoses as well as new medication and am being investigated for another condition, since I was awarded PIP.

Do I write all this down?

I am worried and any advise would be gratefully received.

Yes you write everything down. And in as much detail as you can. Also send a copy of your prescription slip as evidence of your new medications and explain any side effects you get from them.
