Do MH issues get better with age?

I have a sibling who is incredibly harmful to the family and has a history of terrible behaviour and hurt caused. She's (fortunately) divorced herself from the rest of the family (including her young teenage children for many years) but she seems to still find her way back in periodically.
She has a long history of MH problems, I don't know exactly what but she refused any counselling or treatment when I last heard from her about 15/20 years ago.

Anyway, after her latest spiteful and incredibly hurtful behaviour to another sibling, it got me wondering- do MH issues get better with age? Why does a 50 something woman not know how to behave towards others?
Are MH problems really an excuse to be a thoroughly horrible person? Sad
