Has anyone ever requested their mental health notes?

No, I haven't. They can be quite triggering, hence why I have not bothered to apply for PIP etc.

Yes for a relative, but I attended appointments so was written about too. Agree, they can be triggering, I didn't always show them to my relative.

Would advise to only get them if you need them for making a complaint, for example.
