Discharge from Mental health services

I have Bipolar disorder and have been up and down for about 20 years with various hospital admissions under section. I'm pretty well at the moment and being discharged to be under GP care. The Trust that I'm under are pretty awful. They have said they are completely different now and that they deal with urgent re-referrals within 4 hours. However, this hasn't been the case in the past. I have asked in their discharge letter to provide the GP with Medication that they can provide as a stop gap but they won't do this. I'm happy to be discharged but want a proper plan in place. What should I be asking to be put in place?

Do not get discharged without a WRITTEN plan
It will be so hard to get that level of care again, as you say all trusts now have moved there goal posts. Even suicidal doesn’t fit the criteria!!!!

It’s very hard to say what you want
As only you know exactly when in crisis what you need
Think about, if you were in crisis NOW
What would help you???
A safe place to go straight away
Home care(although it’s a waiting game)
Emergency Psych appointment that day
