Am I losing it?

Me and OH have really bad arguments, swearing, hitting, wishing each other dead. He keeps saying that I'm mentally unstable, he says a lot of things to try and break my confidence but then will manipulate it and say I never said that. E.g. you have a fat nose, spotty face, i don't like people with spotty faces. Sometimes things have got so bad that I have actually hit my head on the wall making bumps, recently I just lost it in the car because of the way he would just speak to me like I was thick and stupid, i ended up repeatedly slamming my head on the steering wheel in broad daylight on a roundabout to the point where I had a massive bump on my head. It's so mentally draining. With what I've just said would you say I was mentally unstable or would you say OH is a narcissist and he's the one with all the issues.

Spousal abuse (mental or physical) is simply wrong - regardless of the gender of the person doing it. Sounds like you need to talk to a professional e.g your GP or charity.

It is worth bearing in mind that if you're both suffering form other stresses in life (e.g. cash/over tired/exhausted/work) then these will magnify problems as each seeks a way out of their own dark world - unfortunately you're both the target of the other - and if there are kids, entirely likely to affect them badly too.
