Help with completing mood diary ? Anyone using one please?

I have posted in pregnancy too as I am. ( meant to be) completing one as part of antenatal care due to.previous PND and traumatic births. I can't find the actual diary and can't remember the headings 😦not a good start and I have an appointment tomorrow.
Does anyone else keep a mood diary? Could you help me with what the headings should be please?I remember it had mood and triggers on but nothing else!

DH and I both do, we have slightly different headings personal to us. Mine are stress level, Mood, Motivation and self harm. I give each a score out of 10 each day. It's not a recognised format just one worked out with my CPN that works for me.

Thanks nolongeranxious, I think mine wanted to know triggers for moods wish I could remember the rest. It seems they are lots of different ways of doing it then. I'm anxious and feeling a failure for losing the diary - need to record that Grin
I think there were numbers too now you've jogged my memory so will go back and add those in ( I know that's not the right way to.do it).
Do you find it helpful? So far it's helped me a little in rationalising some thoughts I think.

We use ours to monitor things and try to spot when we are struggling early so that we can do something about it/seek help before we hit crisis point. It's been useful for that and I think making us more aware of our mental states and what things cause moods to dip or to improve.

Sounds good. Hard to start the habit but it does sound useful.
