Advice needed re filling in occup health form re treatment for anxiety

I've been offered a new job at a further education institute and one of the things I have to fill in is an occupational health form. I went to my GP a few months back because I was feeling anxious and she has put me on beta blockers and referred me for a CBT course. I've never missed a day's work in my life, feel much better than I did when I went to see her and certainly don't think I have a condition at present that affects me doing my job. Do I have to tell them? It would certainly raise my anxiety levels stratospherically not to get this job….What should I do? There's two poss questions it affects - do you have a condition that affects your job, and are you having medication/treatment at present

I'm not sure they can legally ask if you are on medication. I thought they could only ask about things that affect your job under the new equality act.

I have always declared my MH problems and it hasn't ever been a problem, they just ask what they can do to support you.
