Clueless - what can a care coordinator/CPN do?

I have homework, to think of things that my CPN can help with. I can never think of anything, so I thought I'd ask here: what does/can your CPN/care coordinator/SW do to help you?

She helped me draw up a relapse signature, with descriptions of my moods from 1-10, strategies for coping at each stage, and at what point to go to 'code red.' It's been really helpful for me to be able to refer to it when I'm unwell (and esp when I'm in denial about stuff).

PS It's about 3-4 pages long. I've also shared it with my husband and it's in my notes.

That's a good idea, dontrunwithscissors, my plan isn't very detailed and could do with being updated. Thanks.

My CPN made my care plan and my risk and relapse plan with me. She also sorts out any appointments I need with the psychiatrist. We do a type of therapy called compassionate mind which I like.
