I live in North London. I did a telephone survey on behalf of the trust that loooks after me. I have been a patient for 4 years, never had a care plan, no CPN and old information regarding crisis numbers.
Alerted survey to all of this. Phone call just now, had I received all those things that were mentioned over a month ago.
